Alternative Core Strengthening Exercises

Core strengthening is a vital aspect of any low back pain rehabilitation programme.

However a complaint we often hear is core strengthening can often be the most boring part of any rehab programme.

Changing up the exercises to keep your rehab interesting while still maintaining gradual progressions and consistency can help make sure you stay on track to hit your goals.

The following are some less conventional core strengthening exercises to try.

1. Reverse Salamander
2. Stir the pot (Swiss ball)
3. Swiss Ball Roll Outs
4. Banded/Cable Palloff Rotations

-Mid Back Pain-

Pain between the shoulder blades can be caused from lack of use of the rhomboids. People may experience this pain whilst sitting at a desk or working a stationary job. This may lead to poor posture due to inhibited muscles and muscular compensations. Thoracic mobility is important to maintain a good posture. It can be helpful to modify positions, postures or movements that are aggravating.

Itโ€™s also important to consider other aspects of your life, such as stress, diminished sleep quality or quantity, or periods of prolonged inactivity. If you notice that certain lifestyle factors contribute to your symptoms, see if you can modify them. For example, if youโ€™re in front of your phone, computer, and TV for most of the day, can you incorporate a few 10 minute walks to break that up?

Lastly, exercises that get your neck, upper back, and shoulders moving in a variety of ways can be helpful. For example, you could choose one thoracic rotation exercise to be performed for 6-8 reps per side every 2-3 hours during the workday to break up periods of sustained positions or inactivity. The exercises in this video can be performed with little equipment and can be performed throughout the day.

-Chronic Knee Pain-

Knee pain can occur in every day activities such as ascending/ descending the stairs, getting up off the couch, getting out of the car, etc. It could potentially mean that the knee joint is not strong enough to tolerate the load that you are putting it through, ie. the surrounding musculature are weak.

Attached are some exercises that can be performed to strengthen up the knee and reduce the load being put through the knee.

1. Terminal knee extension.
2. Wall sits.
3. Lateral plank raise.
4. leg extension isometric hold.
5. Hamstring slide

Early phase Low Back Pain Rehabilitation

During the early phase of rehabilitation from low back pain, whether this be acute or chronic, is managing pain while maintaining mobility and strength in the lower back.

The following is a mobility routine which you would see during this early phase.

These exercises will help give you a base in order to gradually build your lower backs capacity and strength through the next stages so we can get you back to doing things you currently find painful.

Perform each exercise for 30-45 seconds at a time and repeat 3-4 times throughout the day.

DO YOU STRENGTHEN YOUR HIPS AS A RUNNER?

The hip muscles play important roles in supporting the way you run. Firstly, your hip extensors help to propel you forward when you run. Second, they help to stabilize the lumbopelvic junction as the hip adductors & hip abductors need to co-contract to keep the pelvis level. These very same muscles will also work to stabilize the knee by resisting both knee varus and knee valgus forces. Lastly, the hip flexors function to raise the knee up in front of you prior to planting the foot on the floor.

Tennis elbow (lateral epicondylitis) is a painful condition that occurs when tendons in your elbow are overloaded, usually by repetitive motions of the wrist and arm. Symptoms include pain, burning, or an ache along the outside of the forearm and elbow. If it gets worse it may spread down to the wrist if the person continues the activity that causes the condition.
Below are some of our favourite exercises to perform to help alleviate symptoms:
1. Transverse friction massage
2. Wrist extensor stretch
3. Hammer supination
4. Wrist extension
5. Banded supination

Mid-back mobility

The mid-back (thoracic spine) is an area where movement is commonly restricted in individuals, from office workers to athletes.

The primary movements which occur at the thoracic spine are flexion/extension, side flexion and rotation. The main movement of the thoracic spine is rotation , so itโ€™s important that itโ€™s something thatโ€™s trained regularly, as most of what we do on a daily basis tends to be in a flexion/extension type pattern.

Lack of movement in the thoracic spine can result in compensation elsewhere. For example, if you have an ability to extend through your thoracic spine, it is likely you will have reduced overhead mobility and will compensate by increasing the curve (lordosis) in your lower back.

The above 4 exercises can be incorporated daily as part of a warm up of mid-back mobility routine:

1. Overhead thoracic extensions (can use 2 x chairs if you donโ€™t have dowels)
2. 1/2 kneeling overhead side flexion
3. 1/2 kneeling rotation
4. Counterbalance rotations - focusing on moving arm with the weight in the opposite direction to the leg.

Movements should be performed slowly, holding the end range position.

Squat progressions

Squats are a fundamental movement skill which are essential to be able to perform in order to maintain independence and adequately perform activities of daily living (e.g. sitting down and standing up off the toilet unassisted)

It is known as a compound exercise, as it is a full body exercise, which primarily emphasises the glutes, quads and hamstrings.

For many people a regular body weight squat may become relatively easy once they have performed it many times. In this case, it is important to know how to progress the exercise in order to provide an adequate stimulus to your muscles.

The above are a progression of squats you can work on in order to improve your lower body strength:
1. Chair squat
2. Regular squat
3. Counterbalance squat (holding the weight out further will help you to sit deeper into the squat)
4. Goblet squat

Glute bridge progressions

The glute bridge is a simple, yet effective exercise which primarily works the glutes and hamstrings.

It is a great introductory exercise for those who are not accustomed to resistance training, but as a regular glute bridge gets easy for you (you can do 12-15 reps easily), it is important to progress the exercise to incorporate progressive overload into your training, which is essential to continue getting stronger.

The above are 4 variations of a glute bridge, gradually increasing in difficulty:
1. Regular glute bridge
2. B-stance glute bridge
3. Single leg glute bridge
4. Single leg glute bridge with straight leg raise

Nerve gliders and Radicular pain

Radicular pain is pain caused by irritation at the nerve roots in the neck. Some common causes of this are inflammation at the site of the nerve root or a bulge in the discs in the neck causing compression of the nerve root.

This can be a very debilitating issue as pain is often unrelenting

A simple and effective way of managing this pain is through a nerve slider/floss exercise. These exercises can help manage your pain and along with a graded rehabilitation programme can help solve your issue for good.

To Perform a nerve glider

1. Place your arm at 90 degrees with your palm facing towards the ceiling
2. Slowly point your fingers towards the floor and tilt your head towards your hand
3. Slowly curl your fingers towards your head and allow your elbow to bend slightly
4. Simultaneously tilt your head away from your hand

Pain related to sciatica or sacroiliac (SI) joint issues can be incredibly uncomfortable and frustrating. The active figure-stretch is a convenient fix because you can do it as part of the activity that most commonly irritates the sciatic nerve and deep hip rotators: sitting. When done correctly, the figure-4 stretch not only helps relieve the hip tightness that compresses and irritates the sciatic nerve, but it can also help keep SI joints moving properly.

Figure 4 Stretch

Lie on your back with knees bent, feet flat and in line with your hips
Bring either ankle to the opposite knee forming a figure four (i.e. cross your legs)
Reach through your bent knee and grasp the back of the leg still on the ground and pull it towards your chest
You should feel the stretch in your buttocks
Keep your head and neck are relaxed
Hold the position
Repeat for both sides
