2️⃣ Emotional, Spiritual & Psychological Support: Support goes beyond the physical. This stage focuses on helping individuals and families cope emotionally and spiritually, even while treatments continue.
3️⃣ Enacting the Care Plan: As needs evolve, the care plan is put into action. The focus shifts to comfort, dignity, and managing symptoms with compassion and expertise.
4️⃣ Transitioning to End-of-Life Care: In the final weeks or months, care becomes more intensive. The goal is to ensure peace, comfort and support for both the individual and their loved ones.
5️⃣ Supporting Family & Loved Ones: Palliative care continues after loss. Bereavement support helps families process grief and adjust, with counselling, groups, or simply a listening ear.
