🤱 If you're 6+ months postnatal and still not feeling quite right — trust your instincts. A specialist pelvic assessment might be required.

👩‍⚕️Clinical Summary (with permission):

Patient is 9+ months post-natal following a Grade 3A obstetric a**l sphincter injury (OASI). She has been experiencing occasional sudden and random episodes of faecal incontinence.

Consultant assessment indicated normal findings.

🔍Clinical Findings on Assessment; with focus on scar and muscle function:

Scar tissue noted in the anterior quadrant of the a**l sphincter, forming a tight band.

• Appears to be impairing full and uniform contraction of the a**l sphincter, preventing an effective puckered closure.

• Evidence of gaping at the vaginal introitus, particularly at the level of the posterior fourchette, likely impacted from the scar tissue.

• The scar tissue seems to be mechanically limiting muscle function

Current Manual Therapy Mx:

• Initiated dual approach scar tissue and fascial release therapy, targeting both a**l and vaginal walls simultaneously.

• Simultaneous muscle reactivation techniques to promote re-engagement of the pelvic floor and sphincteric musculature.

👐 Manual Therapy is aimed at:

• Assisting the healing process by encouraging realignment of muscle fibres.

• Improving tissue pliability and reducing restrictive scarring.

• Restoring as much of the pre-injury muscular architecture as possible.

• Supporting effective, coordinated contractions of the a**l sphincter and pelvic floor.

🧠 Did You Know?

- Even with adequate anatomical repair, functional recovery can be delayed or incomplete due to scarring and poor muscle recruitment.

- In some cases, the healing process itself can lead to suboptimal fibre orientation and fascial restrictions, which may require manual therapy and neuromuscular retraining to optimise outcomes.

🇮🇪 International Day for Older Persons 🌍

Today, I had the absolute privilege of sitting as an Expert on the panel — surrounded by Irish patriots who have each, in their own unique way, contributed to our society over the course of their lives. 🇮🇪✨

We celebrated more than just age today. We celebrated resilience, wisdom, experience, and the ongoing importance of supporting our older generations in their health and wellbeing.

💊 Empowering older adults to understand their medications is crucial. It can mean the difference between staying healthy and independent or suffering a preventable fall and injury.

🚽 Understanding that while bowel and bladder incontinence may be common, they are not normal, and they can often be managed or treated with the right support.

🍎 Diet and healthy eating play a vital role — for stronger bones, a healthier heart, and a sharper brain.

🧠 And let’s not forget about general wellbeing — from physical activity to mental and emotional health.

Staying socially connected, mentally stimulated, and physically active (even in small ways) can make a world of difference.

The community spirit today was truly heartwarming. Local businesses showed incredible support, reminding us that we all have a role to play in helping each other age well.

We will all age. It's inevitable. But we must never forget that each of us has contributed, whether in small or large ways, to our homes, our streets, our towns, our counties, and humanity as a whole.

So let’s not shy away as we grow older.

Let’s share our wisdom, offer advice, tell our stories, and live with pride.

My message is simple:
Keep moving — physically, emotionally, mentally. No step is too small.

What an Excellent Conference – Pathways to Progress Medico-Legal Conference

Pathways to Progress conference brought together a unique and essential blend of voices - Judges, Barristers, Plaintiffs, Defendants, Patients, AI and tech experts — all in one room, engaging in frank, respectful discussion.

At the centre of every conversation - The Patient.

The conference didn’t shy away from difficult realities but instead embraced them, allowing multiple perspectives to converge around a shared goal: Improving healthcare outcomes in Ireland.

Hon. Justice Emily Egan during her speech offered a memorable a**logy — describing case defence as a parachute descent, where the objective is to land precisely on the “X” marked on the ground. It’s about guiding the descent, navigating variables, and arriving with control and clarity.

From a women’s health perspective, that metaphor resonates deeply.

For many, the parachute is the woman herself, and the goal — the “X” — is a safe delivery and a healthy baby.

However, the path down is rarely straight. Complications, missed diagnoses, delayed interventions, and systemic gaps can throw the descent off course.

And not every woman lands safely on that “X”.

It’s these discussions, these cross-disciplinary conversations, that will shape safer, fairer, and more effective healthcare systems for women, and for all patients.
