TO ICE OR NOT TO ICE AN INJURY???

Today I received a question on a very controversial and confusing topic in the sports medicine industry -

"Where do you stand on the whole ice for injury thing? It seems like we've had a shift from one extreme (all ice all the time) to the other (NEVER ICE)."

This is due to recent paradigm shift by some in the industry reinforcing that inflammation is a necessary part of the healing process, so why would we want to limit inflammation with ice? Proponents of this belief also argue (and rightly so) that there is no evidence that using ice speeds up the recovery process from injury. However, there is also no evidence that icing delays the injury recovery process either!

So here's my stand on the topic:

I sit in the happy medium. I think for things like muscle tears where there isn't much swelling it's not necessary and may slow healing, but for big swollen joints like knees and ankles where swelling is the limiting factor for ROM (which it often is), that not icing will actually SLOW DOWN recovery as you can't do much rehab without decent range of movement! And there's clearly enough inflammation going on for healing in those swollen joint cases. It's not like ice stops ALL swelling/inflammation!

So in summary, its a topic we need a lot more research on before anyone can speak with any real confidence on it. I look forward to learning more about it over the coming years!
