سيدة تبلغ من العمر 93 عامًا، كانت تعتمد على المشاية للمشي، تعرضت لكسر في الجزء البعيد من عظم العضد، مما أدى إلى عدم التحام الكسر وأفقدها القدرة على المشي لمدة 3 أشهر، حيث اضطرت لاستخدام الكرسي المتحرك.

تمت معالجة الكسر جراحيًا من خلال إزالة الكالس غير الفعّال، تحفيز العظم، تنظيف القناة، وزراعة العظم، مع تثبيت ثابت باستخدام صفيحة تشريحية خلفية-جانبية من نوع 4.5 مم.

بدأت المريضة بتحريك ذراعها فورًا بعد العملية، وهي الآن في طريقها للشفاء وتستطيع استخدام ذراعها مرة أخرى للمشي.

قوة الإرادة والعلاج المناسب يمكن أن يصنعا فرقًا كبيرًا – حتى في عمر 90 عامًا 💪

A 93-year-old woman who relied on a walker for mobility sustained a distal humerus fracture that failed to unite, leaving her unable to walk for three months and confined to a wheelchair.

She underwent surgical treatment involving resection of the nonunion callus, bone decortication, canal drilling, bone grafting, and stable fixation with a 4.5 mm anatomical posterolateral plate.

Remarkably, she began moving her arm immediately after surgery. Now she is healing well and has regained the ability to use her arm for walking support again.

With the right treatment and strong determination — even at 90 years old — recovery is possible 💪

في عمليات استبدال مفصل الكتف الجزئي (Hemiarthroplasty)، نستخدم فحص الـ CT بشكل دقيق لتخطيط الارتفاع المناسب للساق (Stem) قبل العملية. نقيس المسافة بين الحافة العلوية لوتر العضلة الصدر الكبرى (Pectoralis Major) إلى النقطة العليا لرأس المفصل الاصطناعي – والتي تكون عادة على الحافة الخارجية للرأس.

كما نحرص أن لا يتجاوز ارتفاع الرأس الصناعي أكثر من 5 ملم فوق مستوى (Greater Tuberosity)، وذلك بهدف الحفاظ على التوازن التشريحي الطبيعي وإعادة بناء ما يُعرف بـ (Gothic Arch) في صور الأشعة بعد الجراحة – وهو دليل على تموضع صحيح ومتوازن للمفصل.

التخطيط الدقيق يساعد في تحسين نتائج العملية وتحقيق أفضل وظيفة للمريض.

In shoulder hemiarthroplasty, we use CT scans for preoperative planning of stem height. One of the key references is the distance from the upper border of the pectoralis major tendon to the highest point of the prosthetic humeral head—which is typically the lateral edge of the prosthesis. In addition to the height of the Greater tuborosity

We also ensure that the prosthetic head is no more than 5 mm higher than the greater tuberosity, aiming for anatomical restoration.

This careful alignment allows us to reconstruct the "gothic arch" on postoperative X-rays—a reliable sign of proper implant positioning and joint balance.

Preoperative imaging = Better outcomes 💪🦴

🔹 كيفية تحضير سبيسر مؤقت للكتف في حالات الالتهاب أو كمرحلة تمهيدية للجراحة النهائية 🔹

في حالات التهابات مفصل الكتف أو عند الحاجة لتأخير الزراعة النهائية، يمكن تحضير "سبيسر" (spacer) مؤقت من مادة الإسمنت الطبي المخلوطة بالمضاد الحيوي المناسب. يتم تشكيل السبيسر داخل غرفة العمليات، بحيث يحاكي شكل رأس العظمة ويُزرع في المفصل للحفاظ على المساحة وتسهيل الحركة البسيطة.

✅ الهدف من السبيسر:

السيطرة على الالتهاب.

المحافظة على طول الأنسجة والمسافة المفصلية.

تسهيل العملية النهائية لاحقًا.

يمكن تصميم السبيسر باستخدام قالب معدني أو تشكيل يدوي حسب الحالة، مع مراعاة تثبيته جيدًا لمنع التحرك أو التآكل.

🦴 أداة مهمة وفعّالة في مراحل علاج التهابات المفاصل المعقدة وخاصة الكتف.

🔹 How to Prepare a Shoulder Spacer for Infection or Temporary Staging Procedures 🔹

In cases of shoulder joint infection or when a staged approach is needed before definitive arthroplasty, a temporary spacer can be made using antibiotic-loaded bone cement. It is molded intraoperatively to resemble the humeral head and inserted to maintain joint space and soft tissue tension.

✅ Purpose of the spacer:

Infection control، Preserving joint space and limb length and facilitating easier final reconstruction.

The spacer can be shaped manually or using a pre-made mold, depending on the case, and must be securely placed to avoid loosening or wear.

🦴 A practical and effective solution in managing complex shoulder infections.

مريض في الأربعينات من عمره يعاني من كسر في حقّ الكتف (الجلينويد) وجسم لوح الكتف (السكابيولا)، بعد تعرّضه لإصابة عنيفة.

قررنا في الفريق الطبي إجراء تثبيت للكسر باستخدام تقنية متقدمة: رد مغلق للكسر مع التثبيت بمساعدة المنظار arthroscopic-assisted fixation، بهدف إعادة الوضع التشريحي الطبيعي للمفصل مع تقليل التداخل الجراحي وتحقيق شفاء أسرع وتحسن وظيفي أفضل.

التدخل الجراحي سيكون دقيقًا ويعتمد على التصوير المسبق ثلاثي الأبعاد وتخطيط دقيق لموقع الأدوات والتثبيت الداخلي.

A man in his 40s sustained a glenoid and scapular body fracture following high-energy trauma.

We have planned an arthroscopic-assisted fixation after achieving anatomical reduction, utilizing advanced minimally invasive techniques to restore the native joint anatomy while minimizing soft tissue disruption.

This approach allows for precise reduction, stable internal fixation, and optimized functional recovery, especially in complex shoulder girdle injuries.

🔹 استبدال جزئي لمفصل الكتف – تحسن تلقائي في "الخلع الكاذب" 🔹
الصورة الأولى: صورة أشعة قبل العملية تُظهر انخفاض رأس العضد بسبب الخلع
الكاذب الناتج عن الألم بعد كسر معقد في رأس العضد القريب.
الصورة الثانية: بعد 4 أسابيع من الجراحة – رأس العضد متمركز بشكل جيد ، وتم زوال الخلع الكاذب كليًا.

تحسن ملحوظ في الألم والحركة بفضل العلاج المبكر وإعادة التأهيل.

تذكير مهم: ليس كل خلع ظاهري يعني عدم استقرار حقيقي – التشخيص الدقيق والتدخل في الوقت المناسب يُعيد التوازن والتشريح السليم للمفصل.



🔹 Shoulder Hemiarthroplasty – Pseudo-subluxation Resolved 🔹

📸 First image: Post-op X-ray showing inferior humeral head migration due to pain-related pseudo-subluxation following complex proximal humerus fracture.

📸 Second image: 4 weeks post hemiarthroplasty – humeral head is well-centered in the glenoid, and the pseudo-subluxation has resolved completely.

✅ Early passive motion and pain control contributed to rapid functional improvement.

🦴 A good reminder: not all subluxations are true instability — correct diagnosis and timely intervention can restore anatomy and motion.

🦴 خلع مزمن أمامي لعظم العضد القريب بعمر ٤٢ سنة– تم علاجه بنجاح! 💪

تمكنا مؤخرًا من علاج حالة معقدة لـ خلع مزمن أمامي مع كسر في الجزء القريب من عظم العضد، حيث كان يعاني من تشوه واضح في المفصل وألم شديد.

✅ أُجري للمريض استبدال نصفي للمفصل (Hemiarthroplasty)
مع تثبيت (Tuberosities)
، مما ساعد على استعادة استقرار وشكل المفصل.

📸 أظهرت صورة الأشعة بعد العملية أن المفصل في وضع جيد، لكن لوحظ وجود ما يُعرف بـ "الارتخاء الكاذب (Pseudo-laxation)"، والذي يعود إلى ضعف في عضلة الدلتويد — وهو أمر متوقع في الحالات المزمنة المشابهة.

💡 نتوقع تحسنًا تدريجيًا في الوظيفة مع المتابعة الدقيقة وجلسات التأهيل الطبيعي.

إعادة بناء الكتف لا تقتصر على استبدال العظم فقط، بل تهدف إلى استعادة الحركة وجودة الحياة 🙌



🦴 Chronic Anterior Proximal Humerus Fracture Dislocation – Successfully Treated! 💪

We recently treated a challenging case of chronic anterior dislocation of a proximal humerus fracture in a patient 42 year old, male, who presented late, with significant joint deformity and pain.

✅ The patient underwent hemiarthroplasty with meticulous tuberosities fixation, allowing for restoration of joint anatomy and stability.
📸 Post-op X-ray showed a well-positioned prosthesis and reduced joint alignment.

However, we noted a phenomenon known as “pseudo-laxation”, caused by deltoid muscle weakness rather than implant failure — an expected finding in chronic cases like this.

💡 With focused rehab, we anticipate steady functional improvement over the coming weeks!

Shoulder reconstruction is not just about replacing bone — it’s about restoring function and life quality. 🙌

غدًا سنقوم بإجراء تثبيت كسر الكوع المعقد واستبدال رأس الكعبرة لحالة كسر مونتيجيا المعقد. يتميز هذا النوع من الإصابات بكسر في الجزء القريب من عظم الزند مع خلع رأس الكعبرة، مما يتطلب تدخلاً جراحيًا دقيقًا لاستعادة استقرار الكوع ووظيفة الساعد.

Monteggia Fracture: ORIF & Radial Head Arthroplasty Case 🔹

Tomorrow, we’ll be performing Open Reduction and Internal Fixation (ORIF) of the ulna along with Radial Head Arthroplasty for a complex Monteggia fracture. This injury, characterized by a proximal ulna fracture with radial head dislocation, requires precise surgical management to restore elbow stability and forearm function.

💡 Key Surgical Steps:
✅ ORIF of the ulna to restore proper length and alignment
✅ Radial head arthroplasty to replace the damaged radial head
✅ Checking ligamentous stability and ensuring correct radial alignment

الأسبوع الماضي، أجرينا جراحة مراجعة متقدمة لمريض يبلغ من العمر 45 عامًا بعد فشل عملية سابقة لإصلاح تمزق الكفة المدورة. كان لدى المريض تمزق كبير ومتراجع في الكفة المدورة، مما تطلب حلاً جراحيًا متطورًا لاستعادة وظيفة الكتف.

بحمد الله، قمنا بتنفيذ نقل الوتر السفلي للترابيزيوس باستخدام طعوم وتر العرقوب، وهي تقنية تهدف إلى تحسين الثبات، القوة، والحركة لدى المرضى الذين يعانون من تمزقات غير قابلة للإصلاح في الكفة المدورة.

🚨 Successful Revision Shoulder Surgery! 🚨

Last week, we had the opportunity to perform a complex revision surgery for a 45-year-old patient after a failed rotator cuff repair. The patient had a massively retracted cuff tear, which required an advanced solution to restore shoulder function.

We successfully performed a lower trapezius transfer using an Achilles allograft, a technique designed to improve stability, strength, and mobility for patients with irreparable rotator cuff tears.
