האף התאפיין ברוחב משמעותי בשילוב גבנון קל. מטרת הניתוח הייתה לעדן את מבנה האף תוך שמירה מלאה על פרופורציות ומאפיינים גבריים. במהלך הניתוח בוצע צמצום מבוקר של רוחב האף, הן בגשר האף והן בעצמות האף, לצד תיקון הגבנון ושימור מבנה טבעי וחזק של השלד האפי, ליצירת מראה גברי והרמוני.

The nose was characterized by significant width combined with a mild dorsal hump. The goal of surgery was to refine the nasal structure while fully preserving masculine proportions and characteristics. During the procedure, controlled narrowing of the nasal width was performed both at the nasal bridge and the nasal bones, together with correction of the dorsal hump and preservation of a strong and natural nasal framework, maintaining a masculine and harmonious appearance.

הבעיות המרכזיות שזוהו: אף גדול ופורץ קדימה, גבנון משמעותי וסטייה של ציר האף לימין. בנוסף, עור דק במיוחד שהיווה אתגר כירורגי ודרש תכנון מדויק ועדין.
במהלך הניתוח בוצע תיקון הגבנון ויישור ציר האף, תוך שמירה מקסימלית על מבני התמיכה הטבעיים במסגרת גישה משמרת. בשל העור הדק נלקחה פציה טמפורלית, אשר שימשה לעיבוי ולריכוך המעטפת מעל השלד החדש. השילוב בין טכניקה משמרת לשימוש בפציה אפשר שליטה גבוהה בתוצאה, תוך יצירת מראה רך וטבעי.
התוצאה לאחר ארבעה חודשים: אף ישר, מאוזן והרמוני, נשי ומדויק, תוך שמירה על יופיה הייחודי של המטופלת.

Four months following preservation rhinoplasty in a young patient in her early thirties.
The primary concerns identified were a large and projecting nose, a significant dorsal hump, and deviation of the nasal axis to the right. In addition, very thin skin presented a surgical challenge and required precise and delicate planning.
During surgery, the dorsal hump was corrected and the nasal axis was straightened, while preserving the native structural support through a preservation approach. Due to the thin skin, temporalis fascia was harvested and used to provide soft tissue augmentation and gentle camouflage over the newly contoured framework. The combination of preservation technique and fascia grafting allowed for a high level of control, creating a soft and natural appearance.
At four months, the result demonstrates a straight, balanced, and harmonious nose that is refined and feminine, while preserving the patient’s unique beauty.
