👁️ Two patients. Two corneal transplants done decades ago.
Both had advanced keratoconus in their youth.

Today, the challenge remains the same: how to design a lens that respects a fragile graft.
👉 The key issue is always suction. The fitter’s role is to find a balance — the “equilibrium fit” — so the patient can wear the lens safely all day long.

Just some cases to share from today at the clinic.

Still with my overdue post - Last day in London – but what a day.

Just before flying back to Israel (and just before the war broke out), I had the privilege of spending an unforgettable day at Moorfields Eye Hospital , one of the most iconic institutions in global eye care.

👁️‍🗨️ I met again with Ken Pullum .pullum, a world-renowned expert in scleral lens fitting. Observing and sharing complex clinical cases with him is always a true honor.

I also had the pleasure of meeting again , a brilliant and passionate optometrist, who specializes in challenging and advanced lens fittings. I’m certain our paths will cross again in the future.

🌍 Every encounter, every conversation, every shared insight is a reminder of why I keep running forward – to learn more, to give more, and to push the boundaries of what we can offer our patients.
Even though I landed straight into a state of emergency, the energy and motivation from this journey are still fueling me every day.

🇬🇧 Northampton, sunshine, and world-class eyecare – what a day! ☀️👁️

Last month, I had the privilege of spending a full day at the practice of the legendary Brian Tompkins in Northampton. A true masterclass in optometry – from seeing patients together to exchanging thoughts on advanced technologies and innovation in scleral lenses and ocular surface management.

I also had the chance to connect with the brilliant Keyur Patel and other incredible colleagues – an inspiring and energizing experience, both professionally and personally.

And… I must say, I never expected to enjoy Northampton this much (thank you Brian Tompkins )– stunning town, peaceful atmosphere, and yes, we even brought the sunshine with us! 🌤️

Since that day, life has been a whirlwind:
➤ I returned to Israel just in time to witness the start of the conflict with Iran
➤ What was supposed to be a short stop between two major congresses quickly turned into a crisis – with cancelled flights and emergency routines
➤ Sirens, children at home, and patients still needing care under alert
➤ Two new high-tech devices now installed at M’Eye Clinic 🔬
➤ A return to the “routine” with a to-do list longer than ever

So yes – it took me a while to sit down and post… but I’m back now and I’ll be sharing more highlights very soon, including a few (very overdue!) posts.

כשאנשי מקצוע מתחברים מעבר לגבולות – מתרחש קסם.

KC oe RK?
2 différents concept and meaning. One is for Keratoconus, and the other one is for an old surgery in order to get rid of glasses.
This patient was diagnosed with Keratoconus and then her surgeon decided to make this surgery in order to correct her vision and « stop » the KC thanks to the scars.. (not so popular today).
After more than 20 years, she cannot live like this anymore and goes back to 20/20 with scleral lenses!
2 אותיות, מושגים שונים ומאד נדיר לראות אותם ביחד.
אחד בשביל קרטוקונוס והשני מתיחס לניתוח ישן שהיה נעשה על ידי חתכים בקרנית להסרת משקפיים.
בתמונות אלו אפשר לראות קרניות של פציינטית שאובחנה עם קרטוקונוס לפני יותר מ20 שנה ועברה ניתוח כזה קצת אחרי, על מנת לשפר את הראייה. הרציונל היה בזמנו שהצלקות שייגרמו מהניתוח יעשו את הקרנית יותר קשיחה וכך גם יעצרו את הקרטוקונוס המתקדם (כיום פחות מקובל מפני שזוהי גישה שיכולה להיות מסוכנת במיוחד כאשר הקרניות דקות כל כך. )
כיום הפציינטית כבר לא מצליחה להסתדר עם הראיה )מצורף גם תמונה) וצריכה עדשות סקלרליות מתקדמות על מנת לתקן את העיוותים הרבים שנגרמו.

רק ליופי רצינו לשתף אתכם עם המקרה העניין של פציינט עם היסטוריה ארוכה לאחר ניתוח קרנית, גלאוקומה ויובש חריג. הפציינט בן 79 וכבר לא רואה טוב עם משקפיים בעין זו. אין שיפור עם שינוי מרשם ובקושי מגיע לחדות ראייה של 6/90 באיכות לא טובה. עם העדשה פול-סקלרל בקוטר 18.5 הוא אכן מגיע ל6/9-. אנחנו מצפים לעוד שיפור עם הזמן. תהיו מוכנים לעדכונים!

Just for the beauty we wanted to post this beautiful case of patient with a long history of RK, glaucome and dry eye that is not longer able to see well with his glasses. This 79 y.o. patient has no improvement with changing his prescription and can only achieve a bad 20/300 in glasses in his left eye. With this full scleral lens he is now able to see a 20/30-. We are expecting some improvement over time. Stay tuned for the updates!

לפעמים הפציינט יתלונן על כעבי ראש שנובע מיובש בעיניים. לא לשכוח לבדוק לו את הדמעות ואת תפקוד של בלוטות המייבומיאן שלו. יכולים לראות הפתעות מידי פעם.👁️👁️👁️

Somtimes the patient will complain about headaches that can be due to dry eye so don’t forget to check his tear and the meibomian gland function. 👁️👁️👁️
