Tamir Fogel IBCLC Board Certified Lactation Consultant, Mother-Baby Nurse, Mom of 3 littles Caring for others has always been a part of who I am. and Israel.

After becoming a registered nurse in 2008, I discovered my passion for helping mothers and babies. Since then, I have worked as a mother-baby nurse in hospitals in the U.S. In addition, as an internationally board-certified lactation consultant, I have worked with mothers in hospitals, in their homes, and virtually. Most recently, I worked as a lactation consultant at Northside Hospital in Atlanta

, Georgia, USA. Northside Hospital (dubbed the “baby factory”) delivers more babies a year than any other hospital and has the largest lactation department in the U.S. When I became a mother, I experienced many of the unexpected challenges that can arise during labor, birth, and postpartum. I have found myself using an SNS the last few weeks during consults to help moms with breastfeeding. Have you heard of it or tried one before? An SNS is a supplemental nursing system. It is a device that allows your baby to receive extra milk (either expressed breastmilk or formula) through a small tube while nursing at the breast. This helps maintain the breastfeeding bond and stimulates your milk production. How to Use an SNS:
1. Prepare the System: Fill the bottle with milk and attach the tubing.
2. Position the Tube: Place the thin tube alongside your ni**le so it reaches the baby’s mouth.
3. Latch Your Baby: Help your baby latch onto the breast, ensuring the tube is in their mouth too.
4. Feed and Bond: Your baby will receive milk from both the breast and the SNS, ensuring they get enough nutrition while you continue to breastfeed. It can take more than 2 hands at first to figure out how to use it but once you get the hang of it, it can be a game changer for moms with low milk supply, babies with latch issues or premature/low birth weight babies. Every breastfeeding journey is unique. This is me with my third baby. I thought it would go so smoothly just like it did with my second because I was such an experienced mom and lactation consultant . In this picture, you can see the bottle in my hand. I had to switch to pumping and bottlefeeding because I was in so much pain and had cracked and bleeding ni**les My baby had a posterior tongue tie and a very high palate. I had to go see a colleague for a lactation consult and see an ENT to get my daughter a frenectomy. After all of these appointments, it still took many weeks to slowly wean off the bottle and pump and get back to exclusive breastfeeding. It was such an unexpected experience to go through during my maternity leave. I am proud of myself for persevering and accomplishing my goal to breastfeed my baby. Looking back, I’m really happy I went through this and am now able to bring this experience with me as I help other moms on their breastfeeding journeys. It’s ok to ask for help!

So many moms don’t realize that their actions ultimately affect their overall milk production. The first 2 weeks are critical in creating your long term milk supply. As you can see in this chart, we all start at the same place, but the mom that removes milk at least 8 times in 24 hours (early and frequent feedings) has a higher milk production at 1 month than a mom who isn’t able to remove milk that frequently.

Pumping for your baby is a big commitment and no small feat! Hooking yourself up to a machine every 3 hours, cleaning pump parts, labeling and tracking how much milk you have in your fridge or freezer, defrosting milk in advance of feeding, and on and on.

It can be stressful trying to pump as much as your baby eats. Here's one small tip that may help.

"Never wake a sleeping baby". How often have you heard this advice?

Sometimes though you do need to wake your newborn to feed. In the first few weeks to month of life, you want to make sure your baby is nursing effectively every 2-3 hours or at least 8-12 times in 24 hours.
If a baby is having trouble with breastfeeding they can tire out quickly and fall asleep at the breast or sleep to conserve energy because they are not eating enough.

Breastmilk changes to meet your baby's nutritional needs. It is dynamic and composed of so many amazing nutrients.

~ It has vitamins and minerals that support health growth and organ function.
~ It has over 200 complex sugars called oligosaccharides that act as prebiotics, providing your baby with a healthy gut.
~ It has millions of live cells; including white blood cells that help with immunity and stem cells that help can help with organ development and healing.
~ It has tons of proteins that are made up of amino acids. Some of these amino acids, nucleotides, increase in your milk at night and are though to help induce sleep.

These are just some of the unique components of your breastmilk! Any amount you can provide for your baby will be beneficial! You are amazing!

A study done by Medela on pumping showed a 12% increase in milk output when moms implemented "hands on" pumping. This term refers to when you massage your breasts while pumping. The massage and gentle kneading of the breasts helps more milk to be expressed.

Are you having ni**le pain and soreness? Is your baby fussy or crying at the breast? When they latch on, are they gulping, gasping or coughing at the breast and then pulling off?
These are all signs that they are overwhelmed by the fast flow of milk. It is called an overactive letdown (OALD).
There are ways to manage your fast flow of milk so your baby has an easier time with breastfeeding. Check out my post and reach out if you have questions or need help.

Have you ever wondered why you wake up during the night or in the morning and your breasts feel so full? The hormone that controls our milk production is called prolactin and the amount our body produces increases and decreases throughout the day and night.
It actually is produced based on how we used to function as humans long ago when we were hunter-gatherers. Women back then tended to wear their babies on their backs all day while they worked and breastfed while they co-slept all night. Because of this history, our bodies peak in their milk production around 2-3 am and we have our highest amounts of milk from the middle of the night through the morning hours. You may notice that your breasts feel fuller in the morning and less full in the afternoon/evening.
This is definitely hard on us as modern day mothers. What's most important though is to empty your breasts at least once during the night so that your hormone prolactin knows to keep up your milk supply. This will protect your milk supply for your baby.

Colostrum.
Your "first" milk. Also called liquid gold. It has everything your baby needs in exactly the right amounts in the first few days. It is thick and sticky and can come in a variety of colors.
If your infant is having a hard time breastfeeding or you are separated for any medical reasons, you should hand express every 2-3 hours to give to your baby with a spoon or dental syringe. This will also help protect and determine your long term milk supply.

Engorgement typically happens when your milk volume increases between day 2-5 after giving birth. It usually only lasts between 12-48 hours. It can also happen later on in your breastfeeding journey if you miss feedings, are adjusting to less feedings when your baby starts eating more solids, or you are weaning.
Talk to a lactation consultant to help with engorgement management or any other questions you have. We are here for you mama!
