It was an honour to be invited to kickstart the Kerela IRIA mid term CME ‘EXPANSE 2025’ to be held on 25 may 2025 in Pala Kerala organised by kottayam city chapter of IRIA. The organisers wanted to include more and more topics, more than what can be included in one day , so they planned a whole week of online webinars for the 170 plus delegates.
Today was the first day and i was invited to speak on imaging of the fetal face with emphasis on the profile view. Thank you to the organisers, especially Dr Rijo sir, president IRIA Kerala, Dr Amilu, Dr Gomathy maam and Dr Judy mam for this opportunity.

Pleased to be part of IRIA ICRI FETAL Radiology Turf Expansion Session, aimed at systematic teaching of Radiology PG residents.

Today’s session I speak on the importance of screening for Pre eclampsia in all pregnant women even in the second and third trimester. I will try and highlight the difference screening of all pregnancies according to Samrakshan Protocols can make in the monitoring and management of high risk pregnancies with examples from day to day practice and Samrakshan studies and publications.

Today’s session is in syc with the World Pre eclampsia day activities which are being conducted by various stakeholders across all countries in order to improve awareness about this dreaded common condition during pregnancy amongst the general public and Radiologists and to highlight the important role of the Radiologist in managing and monitoring these pregnancies.

Pre eclampsia during pregnancy is a much dreaded complication with disastrous consequences both for the mother as well as for her unborn baby.

It involves raised blood pressure, altered kidney function, altered liver function , problems in eyesight, altered blood clotting mechanisms of the body and reduced growth of the baby.

A simple screening test at the time of the three months ultrasound examination, popularly known as the NT NB scan can help to identify 80 percent of our women who are at risk of this dreaded condition.

These pregnant ladies who are thus found to be at risk for pre eclampsia are then given a small very cheap medication to be taken at bedtime daily. This medicine has been found to be instrumental in either completely preventing development of preeclampsia before 8 th month or delaying its occurrence in upto 80 percent of women who regularly take this medicine This also helps in improving the placental function leading to better birthweights of babies.

Samrakshan is a programme by the Indian Radiological and Imaging Association which aims at promoting better and safer motherhood practices amongst our peers so that more and more families can be benefited.

Ask your radiologist for pre eclampsia screening and risk estimation at the time of NT NB scan positively!!

22weeker came for targeted second trimester scan.
Both kidneys appeared echogenic. Liquor was normal. Fetal urinary bladder was well distended. There were no other fetal anomalies. The fetal growth and dopplers were normal.

Considering possibility of polycystic kidneys, patient was offered genetic testing by amniocentesis with clinical exome and Chromosomal microarray.

The result cane out to be 17 q12 deletion with deletion of as many as 29 genes.

The condition is associated with cystic kidney disease, neuropsychiatric problems and juvenile onset type v diabetes.

It is thus advisable to look carefully for not only the size of kidneys but also their echogenicity.

Also genetic testing should be offered to all patients.

Discussion with fellow radiologists yielded some additional information: two patients who had this 17 q 12 deletion and carried pregnancy had polyhydramnios and progressive increase in echogenecity.

Nov 8 1895 was the day Wilhelm Rontgen discovered X-rays and changed the way we see our world forever.
Today we are celebrating the 13th International Day Of Radiology as we all join together on this day to recognise and promote the essential role that radiology plays in the patient journey.

On this day, we promote the seismic impact of radiology on medicine – earlier detection, fewer surgeries, and game-changing treatment outcomes. It is an opportunity, not only to promote these benefits amongst our colleagues in healthcare and the public at large, but to reflect on them ourselves as radiologists.

In this journey all imaging professionals including our radiographers, technicians, typists and staff members work as a team as we ensure optimum patient care.

We at Samrakshan today urge all practising fetal radiologists to adapt the Samrakshan protocols in day to day obstetric imaging, which can be game changer in prevention and management of PE and growth disorders and thus realise the important role we can play in a womans obstetric journey.
