Recently on 30th and 31ST August, The Ahmedabad Arthroscopy Academy (which is an academic group of 7 arthroscopy surgeons) organized 4th Arthroscopy Symposium at Ahmedabad, where I had the privilege to demonstrate few surgical technique in front of large audience of orthopedic surgeons from various parts of Gujarat and India.

With these three amazing Kabaddi players from Maharashtra 🇮🇳, all currently with Sports Authority of India. Each one has faced the challenge of ACL injury, undergone reconstruction surgery, and is now back on the mat 🏋️‍♂️🏃‍♂️🔥

ACL reconstruction in a hyperextension and lax knee presents unique challenges and requires careful surgical planning. Here are some key considerations:

1. Preoperative Assessment

Identify the cause of hyperextension: It could be congenital ligamentous laxity, quadriceps dominance, or secondary to previous injuries.

Check for generalized ligamentous laxity: Beighton score can help assess systemic hypermobility.

Evaluate associated injuries: Meniscal insufficiency, PCL laxity, and posterolateral corner (PLC) injuries can contribute to knee hyperextension.

2. Graft Choice & Tunnel Positioning

Autograft vs. Allograft:

Patellar tendon graft may provide better stability in hyperlax knees.

Hamstring grafts should be tensioned adequately to prevent postoperative laxity.

Femoral tunnel placement:

Avoid excessively anterior placement, as it can lead to graft impingement and loss of flexion.

A slightly more posterior femoral tunnel (in the anatomic footprint) helps restore normal knee biomechanics.

Tibial tunnel positioning:

In hyperextension-prone knees, a slightly posterior tibial tunnel position (without overdoing it) can help avoid graft stretching and anterior knee subluxation.

3. Graft Tensioning & Fixation

Tension the graft in slight flexion (20–30°) to avoid excessive tightness in extension.

Fixation should be strong, especially in cases of generalized laxity. Adjustable suspensory fixation or interference screws can be considered based on graft type.

4. Addressing Hyperextension

Physiotherapy Prehabilitation: Strengthening of hamstrings and posterior chain muscles can help reduce hyperextension preoperatively.

Postoperative Bracing: A hinged knee brace with extension limitation may be used initially to prevent excessive stretching of the graft.

घुटने से क्लिक करने की आवाज़ क्यों आती है?

घुटने से क्लिक करने की आवाज़ कई कारणों से आ सकती है, जो सामान्य शारीरिक प्रक्रियाओं से लेकर कुछ चिकित्सीय स्थितियों तक हो सकते हैं।

1. सामान्य कारण (शारीरिक प्रक्रिया)

कैविटेशन (Cavitation): जोड़ के तरल में छोटे गैस के बुलबुले टूटने से पॉपिंग या क्लिकिंग की आवाज़ आ सकती है (उंगलियों के जोड़ चटकाने की तरह)।

टेंडन या लिगामेंट का हिलना: घुटने के आसपास के टेंडन या लिगामेंट हड्डी के ऊपर से गुजरते समय क्लिकिंग की आवाज़ कर सकते हैं, खासकर अगर वे तंग हों या गलत स्थिति में हों।

मेनिस्कस (Meniscus) का हिलना: घुटने के अंदर मौजूद मेनिस्कस का सामान्य रूप से खिसकना कभी-कभी क्लिकिंग की आवाज़ कर सकता है, बिना किसी दर्द के।

2. रोग संबंधी कारण (ध्यान देने योग्य स्थितियां)

मेनिस्कस टियर (Meniscus Tear): अगर मेनिस्कस फट जाता है, तो चलने या घुटने मोड़ने पर क्लिकिंग के साथ दर्द, सूजन या घुटने के लॉक होने की समस्या हो सकती है।

कार्टिलेज डैमेज (Chondromalacia Patellae): घुटने की हड्डी के नीचे मौजूद कार्टिलेज के नरम पड़ने से घर्षण या क्लिकिंग की आवाज़ आ सकती है।

पटेलोफेमोरल सिंड्रोम (Patellofemoral Syndrome): अगर घुटने की कैप ठीक से संरेखित नहीं होती, तो यह क्लिकिंग और दर्द का कारण बन सकता है।

लूज बॉडीज (Loose Bodies): हड्डी या कार्टिलेज के छोटे टुकड़े घुटने के जोड़ में घूम सकते हैं, जिससे क्लिकिंग या लॉकिंग की समस्या हो सकती है।

लिगामेंट इंजरी (ACL, PCL, MCL Tear): लिगामेंट की चोट से घुटने में अस्थिरता और चलने पर क्लिकिंग की समस्या हो सकती है।

अगर क्लिकिंग के साथ दर्द, सूजन, अस्थिरता या लॉकिंग हो
