First-Time ever in Gujarat:

Happy to share performing
Cone-beam CT (CBCT) guided RP-RBUS assisted bronchoscopic cryobiopsy successfully done at Epic Multispecialty Hospital, Ahmedabad with insightful case courtesy from Pulmo collegue Dr. Yash Rana.

Case vignette:
A lady in her fifties with back ache & easy fatigue for 3 months was found to have osseous lytic lesions on MRI Spine. Subsequent PET scan detetected 2.6cm x 2.0cm intensely avid right upper lobe nodule where RB2aii branch was directly leading with SUV max 17.1 along with PET positive subcarinal mediastinal lymphadenopathy.

Decision making:
As the lesion is invisible bronchoscopically & also difficult to detect through C-arm fluoroscopy due to its size & location, CBCT was choosen as it gives real time tool in lesion confirmation.

As lung biopsy will provide more tissue than needle aspirate, the procedures were combined for need of molecular analysis.

Procedure:
Linear EBUS TBNA sampling from subcarinal lymphnode with ROSE followed by CBCT guided radial probe EBUS assisted cryobiopsies were done with thin scope successfully with confirmation of tool in lesion. ROSE confirmed malignancy. Detailed histopathology awaited.

Advantage over conventional CT guided trans-thoracic lung biopsy:
As trans-thoracic CT guided biopsy may have more than 25% chance of creating pneumothorax in bronchocentric lesion like this case, bronchoscopic modality carries less than 5% risk with providing superior bleed control.

Why a milestone & historic event?
With first-ever case, Gujarat has become the one of few Indian states offering this advanced technology.

CBCT Bronchoscopy era holds exciting future of Pulmo-Onco adding 4th dimension to existing (Chemo/Radio/Surgery) ones, to offer therapeutic radio-frequency ablations of lung tumors.
