Epilepsy needs no introduction, it is the second most common and frequently encountered neurological condition causing heavy burden on patients, families, and even healthcare systems. If it is drug resistant it is even more devastating. Drug Resistant Epilepsy or DRE is a condition where 2 or more epilepsy medication does not work any more on seizures.
VNS (Vagal Nerve Stimulation) is one of the surgical treatment options for DRE- Drug resistant (People who continue to have seizures despite trying two or more medications). Besides seizures, is also associated with other comorbid conditions such as loss, increased risk of accidents/injuries, frequent hospitalisations, as well as compromised of patients and their families, including social stigma.

The classic ketogenic diet (KD) is a special high-fat, adequate protein and low-carbohydrate diet which is one of the treatment options to help control seizures in people with drug resistant epilepsy.

𝗔 𝗸𝗲𝘁𝗼𝗴𝗲𝗻𝗶𝗰 𝗱𝗶𝗲𝘁 "𝗿𝗮𝘁𝗶𝗼" 𝗶𝘀 𝘁𝗵𝗲 𝗿𝗮𝘁𝗶𝗼 𝗼𝗳 𝗳𝗮𝘁 𝘁𝗼 𝗰𝗮𝗿𝗯𝗼𝗵𝘆𝗱𝗿𝗮𝘁𝗲 𝗮𝗻𝗱 𝗽𝗿𝗼𝘁𝗲𝗶𝗻 𝗴𝗿𝗮𝗺𝘀 𝗰𝗼𝗺𝗯𝗶𝗻𝗲𝗱.
For people who cannot follow strict 4:1 ratio due to any reasons, modified Atkin’s diet (MAD) or Low glycemic index therapy (LGIT) with a less stricter ratio can be considered.

- 𝘋𝘪𝘴𝘤𝘰𝘯𝘯𝘦𝘤𝘵𝘪𝘰𝘯 𝘴𝘶𝘳𝘨𝘦𝘳𝘺: if seizure focus cannot be localised or cannot be removed, that part of the brain is disconnected from the other regions of the brain to prevent the spread of the seizures and thereby reducing disabling seizures
- 𝘈𝘣𝘭𝘢𝘵𝘪𝘷𝘦 𝘱𝘳𝘰𝘤𝘦𝘥𝘶𝘳𝘦𝘴: such as stereotactic laser ablation and stereotactic radiosurgery offer minimally invasive alternatives to open surgery.

Outcomes depend largely on type of epilepsy surgery and the brain areas involved.
For instance, around 70% who have temporal lobe epilepsy become seizure-free after the resective surgery, and for a further 20% their seizures are reduced.

𝗦𝘁𝗮𝘁𝘂𝘀 𝗘𝗽𝗶𝗹𝗲𝗽𝘁𝗶𝗰𝘂𝘀 - It is a Neurological emergency!

When a seizure lasts for 5 minutes or more, or if repeated seizures occur for 5 minutes or more without return of normal level of consciousness in between the seizures, it is referred to as Status Epilepticus.

Most seizures stop spontaneously in 2 minutes or less, but if it goes on for 5 minutes or more, the chances that it will stop spontaneously reduces significantly. A seizure lasting for 30 minutes or more may start causing irreversible damage to the brain.

One of the most common causes is low seizure-medication levels in the blood, which commonly is due to 𝘮𝘪𝘴𝘴𝘦𝘥 𝘮𝘦𝘥𝘪𝘤𝘢𝘵𝘪𝘰𝘯 𝘥𝘰𝘴𝘢𝘨𝘦𝘴.
Medication compliance plays a key role in reducing chances for developing Status Epilepticus.

In case of a prolonged seizure (lasting for 5 minutes or more), patient should be brought to the nearest hospital for immediate medical attention.

It is a test that records electrical activity of the brain.

‣ During this procedure, electrodes consisting of small metal discs with thin wires are pasted onto your scalp. These electrodes detect tiny electrical charges that result from the activity of your brain cells.

1 in 3 epilepsy patients have DRE, quality of life is significantly impaired due to uncontrolled seizures and associated illnesses. Early diagnosis and appropriate treatment may improve outcomes.
