Over last 2 decades, Approach to Management of ILD is changed dramatically. HRCT Chest has played a key role in this development. Newer Antifibrotic and usage of existing immunosuppressant for this disease have changed course of disease provided approach is disease specific.
But many a times inspite of thorough clinical radiological evaluation diagnosis remains non specific for ILD.
Hence Histopathological diagnosis is mandatory for further decision making in such cases.
Before few years surgical lung biopsy (VATS ) was the only option which caries high morbidity and mortality. Transbronchial lung biopsy (TBLB)with regular Forceps is unable to provide adequate tissue for diagnosis.
TBLB with Cryo probe is solution for this.
Though initial experience had showed higher complications and lower yields, Improvement in procedural techniques and safety measures helped to achieve yield comparable to surgical lung biopsy without much complications.
Now ,TBLB with cryoprobe no more experimental but it’s a standard tool for management of ILD.
