TUBERCULOSIS OF THE TEMORAL BONE (EAR)
Miraculous hearing outcome in a young patient with tuberculosis of the temporal bone
Female in mid 20s, referred for facial palsy (HB grade 4/5) developing around 2 months after tympanomastoid surgery.
Revision surgery was done with decompression of the horizontal segment of facial nerve.
Grossly polypoidal tissue involving EAC, mastoid and middle ear with extension to basal turn of cochlea and vestibule.
Canal wall down surgery with cavity obliteration was done and temporalis fascia used as graft.
9 months of anti-tubercular treatment was given following surgery as histopath showed features of tuberculosis.
6 months after surgery, patient improved to HB grade 2.
Pleasantly surprising was the hearing gain inspite of gross inner ear involvement by the disease (BERA confirmed serviceable hearing).
