Mechanical thrombectomy for tandem occlusion.

This gentleman had sudden paralysis of the left half of the body. He was rushed to the hospital by relatives and brought within 2 hours. CT scan of the brain did not show any brain hemorrhage and the clot busting drug (thrombolysis) was given promptly by the referring center. However, he deteriorated further following thrombolysis.

This kind of patients carry very high risk for death/ permanent disability if further treatment is not offered beyond thrombolysis.

Repeat CT scan with CT angiography revealed occlusions at two levels in the artery supplying blood to the right half of the brain. He was shifted to a thrombectomy capable center and procedure performed as quickly as possible.

We could open up the occluded arteries and the patient was discharged in 5 days. Then, he comes WALKING to OPD for the follow up!!

This kind of outcome is never possible without team work- referring doctors, ER staff, cathlab staff, ICU team.

This young gentleman with cyanotic heart disease had come to emergency department with headache, seizures and right hemiplegia. Considering this to be the brain abscess, empirical antibiotics were started promptly and early decompression, surgical excision was done. Microbiology showed Nocardia species, antibiotics were modified accordingly. The patient made a dramatic recovery and was able to walk without support in two weeks.
