Old age patient with massive hemetemesis and malena found to have MW year and DU. Bleed started during gold probe cautrization. Controlled by Glu injection

50 year patient with malena for last 5 days presented with anemia with HB 6 gm. Patient was on NSAID and anti platelets. UGI Scopy show gastric ulcer with visible vessel. 1 ml Glu injected followed by gold probe cautrization.

65 year old female admitted with 7 days of malena. UGI Scopy done before 3 days shows suspected ampullary bleed. Patient admitted with severe pallor and weakness. CT angio was done suspecting hemobilia or pancreatic bleed but was normal. Repeat UGI Scopy done which showed duodenal deulafoy lesion. Hot biopsy cautrization done. Patient discharged next day. Procedure would be easy if side viewing Scope was available during procedure.

50 year male post Bariatrics surgery (sleeve gastrectomy) 5 years came with complain of vomiting and fullness in abdomen for 3 days not responding to medications. Endoscopy shows phytobezor in stomach occupying reduced capacity stomach. Bezor was broken with scope and saline flush. Repeat Scopy after 2 days shows clear stomach.
Patient had history of eating one full coconut followed by glass of coffee which formed bezor.
Phytobezor is made of undigested fibers and food particles.

Massive Variceal bleed from mid esophagus. Mid esophageal variceal bleed r difficult to manage specially when varices r small like in this case.

Banding esophageal variceal puncture site is tricky as if it’s missed then bleeding will obscure your vision. Should be done only when varix is large.

Removing impacted stone at ampulla followed by removal of multiple stones from common bile duct.

Hemetemesis due to esophageal variceal bleed with visible site of recent bleed-nipple. Glu injection done followed by banding of remaining varices.

First time diagnosed cirrhosis of liver with massive hemetemesis form fundic varices. 3 ml Glu injected.
