A 40-year-old female presented with a liver mass which, on detailed evaluation and subsequent surgical excision, was diagnosed as 𝐌𝐞𝐭𝐚𝐬𝐭𝐚𝐭𝐢𝐜 𝐁𝐞𝐧𝐢𝐠𝐧 𝐋𝐞𝐢𝐨𝐦𝐲𝐨𝐦𝐚 (𝐌𝐁𝐋) 𝐢𝐧𝐯𝐨𝐥𝐯𝐢𝐧𝐠 𝐭𝐡𝐞 𝐥𝐢𝐯𝐞𝐫 — an exceptionally rare entity.

Despite the word “metastatic,” this condition behaves very differently from conventional malignant metastasis.

Metastatic Benign Leiomyoma is a rare smooth muscle tumor originating most commonly from the uterus, where histologically benign leiomyoma cells spread to distant organs such as the lungs, pelvis, lymph nodes, and 𝐯𝐞𝐫𝐲 𝐫𝐚𝐫𝐞𝐥𝐲, 𝐭𝐡𝐞 𝐥𝐢𝐯𝐞𝐫.

The procedure lasted approximately 4 hours, and the postoperative recovery was smooth. She was discharged in stable condition on Post-Operative Day 4.

The journey, however, is still ongoing, as evaluation and management of the primary uterine source remain essential.

Cases like these remind us that not every metastatic-looking lesion is malignant — and definitive diagnosis often lies in histopathology.

A 71-year-old gentleman with end-stage liver disease due to MASLD presented to us at in a critical condition. While age is often perceived as a limiting factor, his family chose hope over hesitation.

In an extraordinary act of courage and love, his son stepped forward as a living donor.

This was a technically demanding transplant due to complex graft anatomy requiring meticulous planning and reconstruction:

• 𝐓𝐲𝐩𝐞 𝟑𝐚 (𝐂𝐡𝐨𝐢) 𝐛𝐢𝐥𝐢𝐚𝐫𝐲 𝐚𝐧𝐚𝐭𝐨𝐦𝐲 — 𝐝𝐮𝐚𝐥 𝐛𝐢𝐥𝐞 𝐝𝐮𝐜𝐭𝐬

The surgery was successfully performed under the leadership of and with excellent surgical support from 𝐃𝐫. 𝐆𝐮𝐫𝐮𝐩𝐫𝐚𝐬𝐚𝐝 𝐒𝐡𝐞𝐭𝐭𝐲. Despite the complexity, the intraoperative course was well-managed, requiring only 𝟐 𝐮𝐧𝐢𝐭𝐬 𝐨𝐟 𝐏𝐂𝐕 𝐭𝐫𝐚𝐧𝐬𝐟𝐮𝐬𝐢𝐨𝐧.

The post-operative phase was stormy and demanded continuous vigilance. However, with the relentless efforts of our 𝐂𝐫𝐢𝐭𝐢𝐜𝐚𝐥 𝐂𝐚𝐫𝐞(Dr. Jay Kothari and Dr ), 𝐇𝐞𝐩𝐚𝐭𝐨𝐥𝐨𝐠𝐲 (), 𝐀𝐧𝐚𝐞𝐬𝐭𝐡𝐞𝐬𝐢𝐚 (Dr and Dr Bhavesh), 𝐚𝐧𝐝 𝐓𝐫𝐚𝐧𝐬𝐩𝐥𝐚𝐧𝐭 𝐒𝐮𝐫𝐠𝐞𝐫𝐲 𝐭𝐞𝐚𝐦𝐬 (Dr Chirag Desai, Dr. ) , every challenge was systematically addressed.

We discharged the patient on Post Operatove Day 25 in a stable and recovering condition — a true testament to the power of teamwork, precision surgery, and unwavering determination.

Transplant vs Resection in Child A Cirrhosis with Resectable HCC – The Eternal Debate

In patients with Child A cirrhosis and resectable Hepatocellular Carcinoma (HCC), the question remains:

👉 Should we offer Liver Transplant or go for Resection?

Both options are valid—but fundamentally different.

🔹 Liver Transplant
• Treats both tumor and underlying cirrhosis
• Lower recurrence rates
• But comes with higher short-term morbidity/mortality
• Limited by donor availability and logistics

🔹 Liver Resection
• Technically straightforward in well-selected Child A patients
• Lower immediate risk
• But higher recurrence rates due to underlying liver disease

This is not just a surgical decision—it’s a patient-centered decision.
