One of the first questions that comes to every expecting parent’s mind after a positive pregnancy test is – “When will we hear our baby’s heartbeat?”

In this video, Dr. Pooja Pol, Fetal Medicine Specialist at SonoFitz, Ahilyanagar, explains exactly when the baby’s heartbeat begins and how it can differ based on your menstrual cycle.

If your periods are regular, the baby’s heartbeat can usually be seen on an ultrasound around 6 to 7 weeks of pregnancy. This is typically confirmed during the early pregnancy scan.

But if your periods are irregular, the dating of your pregnancy might not match your last menstrual period. In such cases, the heartbeat might be seen a little later, around 7 to 8 weeks, depending on ovulation and conception timing.

The heartbeat is a major sign of a healthy early pregnancy, and fetal medicine doctors use advanced techniques to ensure it’s detected at the right time.

When is Down Syndrome detected?
Down Syndrome can be screened during the first trimester, usually between 11 to 13 weeks through a NT Scan and Double Marker blood test. Together, this is called the First Trimester Combined Screening.

Which sonography helps detect it?
The NT Scan (Nuchal Translucency) checks fluid at the back of the baby’s neck and other markers like nasal bone, tricuspid flow, and ductus venosus to assess risk.
Screening Methods:
In low-risk pregnancies, this screening along with blood tests like Double Marker or NIPT helps estimate the risk of Down Syndrome.

In high-risk pregnancies (advanced maternal age, family history, previous baby with genetic condition), more detailed tests like NIPT or Confirmatory invasive tests (CVS or Amniocentesis) may be suggested.

सोनोग्राफीमध्ये दिसणारी प्रत्येक त्रुटी किंवा व्यंगं गंभीर असतेच असं नाही. खाली काही महत्त्वाचे मुद्दे दिले आहेत:

- काही व्यंगं सौम्य (mild) असतात – जसं की हलकीशी किडनीमध्ये सूज, थोडं पोटात पाणी, इत्यादी – जी बाळ जन्मल्यानंतर आपोआप सुधारतात किंवा किरकोळ औषधांनी बरी होतात.

- काही व्यंगं गंभीर असू शकतात – जसं की मेंदूमध्ये पाणी (hydrocephalus), हृदयाची त्रुटी, ओठ फाटलेले (cleft lip), यांसारख्या अटी ज्यासाठी जन्मानंतर तत्काळ उपचाराची गरज असते.

- कधी काही त्रुटीची पुष्टी करण्यासाठी विशेष तपासण्या कराव्या लागतात – जसे की:

Amniocentesis – गर्भजलाची तपासणी

CVS (Chorionic Villus Sampling) – गर्भाशयातून टिश्यू नमुना घेणे
या चाचण्या अनुवांशिक व्यंगं आहेत का हे निश्चित करण्यासाठी केल्या जातात.

Myth 1: Transvaginal Sonography (TVS) causes bleeding
Fact: TVS involves gently inserting a probe into the vaginal canal – it does not even touch the uterus. So if bleeding occurs, it is not due to the scan itself. TVS is a safe and routine test done in early pregnancy.

Myth 2: Frequent sonography leads to developmental problems in the baby
Fact: Sonography uses ultrasound waves, not harmful radiation. Extensive global research has proven that ultrasound is safe for both the mother and the baby. It does not cause developmental issues.

Myth 3: 3D/4D scans are necessary for all pregnancies
Fact: While 3D/4D imaging may help visualize the baby’s face or identify certain defects more clearly, it is not essential for diagnosis. The most important and detailed scan is the 2D sonography, which doctors use to check fetal organs and development.

Myth 4: A normal anomaly scan means the baby is 100% normal
Fact: The anomaly scan, usually done between 18–22 weeks, checks if key organs have developed properly. However, it is still a screening test and cannot guarantee that everything is perfect. Some issues may only be identified later or after birth.

Myth 5: The delivery date given in sonography will be exact
Fact: The EDD (Estimated Due Date) given in sonography is just an approximate date based on a 9 months, 9 days calculation. Only 5–8% of women deliver on this date. Others deliver a few days before or after.

Myth: Fetal Medicine is only for high-risk pregnancies

Myth: Fetal Interventions are unsafe or experimental
Fact: While interventions carry some risk (like any medical procedure), they are not unsafe or experimental. They are done by highly trained fetal medicine specialists with utmost precision and care.

There are two major types of birth defects:

1. Genetic (Chromosomal)
2. Structural (Physical)

Detecting Genetic Defects:
Using the NT Scan (11–14 weeks), fetal medicine specialists assess specific markers such as Nuchal Translucency (NT), skin thickness, and nasal bone to screen for conditions like Down syndrome and other chromosomal abnormalities.
Advanced blood tests like the Double Marker Test and NIPT (Non-Invasive Prenatal Testing) further help estimate the baby’s risk.
If a previous child had a genetic condition, confirmatory testing may be advised to ensure early diagnosis in the current pregnancy.

Detecting Physical Defects:
During the NT scan (1st trimester), fetal medicine experts also examine the baby’s physical organs. Around 60–70% of structural defects can be detected as early as the 3rd month.
By the 5th month (18–20 weeks anomaly scan), around 80–90% of major anomalies can be identified. Every organ—brain, heart, spine, face, limbs—is studied in detail to check for shape, size, and proper development.
