I am delighted to share that on July 17, 2025, I was invited as an Operating Faculty at Endocon Jalna 2025, where I demonstrated my unique technique of Laparoscopic Hystero-Sacropexy with Pelvic Floor Repair for a 3rd-degree uterine prolapse in a young patient.

The procedure received immense appreciation from renowned stalwarts, teachers, and delegates for its precision and innovation.

A 25 years Female G3P1L1A1 referred to Kalavati hospital with continuous pv bleeding since 15 days
LMP- 01-02-25

PMC- 3-4 days/ 28days/ regular/ average flow

O/H - G2P1L1A1
1st - FTND, female baby 6 years A & W
2nd- MTP
3rd- Present pregnancy MTP done 15 days back

USG- Endometrial hyperplasia? RPOCS
CT scan- AV malformation

Plan- Laparoscopic ligation of uterine arteries and mesosalpingial vessels with hysteroscopic excision of mass

Surgical Steps Painting and draping done. Catheterization done.

· 10 mm supraumbilical port inserted with direct trocar entry technique. Slow pneumo-peritoneum has started with co2 flow rate of 3 liter per minute. Once intra-abdominalpressure reached 14 mm of hg, flow rate increased to maximum mode

· 10 mm / 30 degree telescope introduced through 10 mm port. Insertion site and peritoneal cavity inspected. Patient has given modified trendelberg’s position.

· Findings were as follows:: The uterus was bulky in size. B/l mesosalpingeal vessels looked dilated and tortous in size. both tubes and ovaries were normal.rest abdomen WNL.

· 5.5mm lateral port inserted two on left side of optic port and one on right side in mid-clavicle line by following the BASE-BALL DIAMOND concept.

· B/l lateral pelvic wall dissected , uterine artery at origin coagulated .

· B/l meso-salpingeal vessels ligated with vicryl no1.

HYSTEROSCOPY:

· 2.9/30 degree telescope used for hysteroscopy.

· Hysteroscopy started with NO-TOUCH TECHNIQUE.

· Cervical canal, endo-cervical mucosa looked normal.

· At the level of Internal- os negotiated easily.

On entering uterine cavity a mass / ?RPOC of 2 x 1.5 cm in size arising from posterior wall of uterus.

· Rest of Endometrium looked pink , proliferative, and smooth.

· uterine fundus , b/l tubal ostia looked normal.

· Both lateral uterine wall looked parallel, and the cavity was normal in size.

· AV malformation/ RPOCs WAS RESECTED WITH BIPOLAR RESECTOSCOPE. SPECIMEN RETRIEVED WITH O**M FORCEP. Send for HPE.

The procedure was uneventful
