बच्चों का जन्म हमेशा खुशियां लाता हो, यह पूरा सच नहीं है। कुछ बच्चों में जन्म से ऐसी विकृति हो जाती है, जिससे उन्हें बचा पाना मुश्किल हो जाता है। उत्तर प्रदेश के सुल्तानपुर में एक ऐसे ही बच्चे का जन्म हुआ है, जिसका दिल उसके शरीर के बाहर निकला हुआ है। डॉक्टरों के मुताबिक, वह 'एक्टोपिया कॉर्डिस' नाम की एक दुर्लभ बीमारी से पीड़ित है। डॉक्टरों ने यह भी बताया कि उसके तीन दिन से ज्यादा जिंदा रहने की उम्मीद न के बराबर थी, लेकिन छह दिन बाद भी वह जिंदा है।
बता दें कि यह दुर्लभ बीमारी दस लाख लोगों में से सिर्फ 8 को अपनी चपेट में लेती है और इससे पीड़ित 90 फीसदी बच्चे जन्म के तीन दिन के भीतर दम तोड़ देते हैं। फिलहाल इस बच्चे के इलाज के लिए उसके माता-पिता परेशान हैं क्योंकि वे गरीब हैं और ऑपरेशन में खर्च ज्यादा है।

दुर्लभ बीमारी है 'एक्टोपिया कॉर्डिस'
'एक्टोपिया कॉर्डिस' को दुर्लभ बीमारी में से एक माना जाता है। यह एक जन्मजात विकृति है, जिसमें असामान्य रूप से दिल छाती के बाहर या तो आंशिक रूप से या फिर पूरी तरह स्थित होता है। अधिकांश मामलों में दिल गर्दन के नीचे या फिर पेट के आसपास या छाती के बाहर ही निकल आता है।
- इस बीमारी के साथ जन्में बच्चे संक्रमण, हाइपोएक्सीमिया या हार्ट फेल्योर के चलते तीसरे दिन ही मर जाते हैं।

चार तरह की होती है यह बीमारी-
सर्वाइकल- इसमें दिल गर्दन से बाहर निकला होता है।
थोरेसिक- इसमें दिल गले से बाहर निकला होता है।
थोरेकोएब्डॉमिनल- इसमें दिल छाती और पेट के बीच के हिस्से से बाहर निकला होता है।
एब्डॉमिनल- इसमें दिल अपनी जगह होने के बजाय पेट की कैविटी के बीच होता है।

Two Drugs that Hit One Target Show Efficacy against Metastatic Breast Cancer

Combining two drugs that target the HER2 protein, trastuzumab (Herceptin®) and pertuzumab (Perjeta™), with chemotherapy is a new treatment option for women with HER2-positive metastatic breast cancer, according to results from a large clinical trial.

The initial results of the phase III CLEOPATRA trial showed that combining both of the HER2-targeting agents with the chemotherapy drug docetaxel as an initial treatment led to a 6-month improvement in progression-free survival compared with treatment with docetaxel and trastuzumab alone. The results were presented on December 7, 2011, at the San Antonio Breast Cancer Symposium Exit Disclaimer (SABCS) and published the same day in the New England Journal of Medicine.

Targeting HER2 with two different drugs has shown promise in multiple trials, said the trial’s lead investigator, José Baselga, MD, PhD, of Harvard Medical School and the Massachusetts General Hospital, during an SABCS press briefing. “I think dual HER2 blockade is coming soon, and it will be in our daily practices.”

Although both drugs target the HER2 protein on the surface of cancer cells, they do so in different ways. Laboratory studies have indicated that the drugs may have a synergistic effect on HER2-positive tumors, which, Dr. Baselga explained, are “addicted” to HER2 signaling.

More than 800 women were enrolled in the randomized trial; half received all three drugs, and half received trastuzumab and docetaxel, a standard first-line treatment for women with metastatic HER2-positive breast cancer, plus a placebo. Median progression-free survival was 18.5 months in the three-drug arm and 12.4 months in the two-drug plus placebo arm. More women who received the three-drug combination experienced substantial shrinkage of their tumors than women who received trastuzumab, docetaxel, and the placebo.

Trastuzumab has been associated with significant cardiac side effects in some women, but no increase in such side effects was seen in women in the trial who received both HER2-targeted drugs.

The initial analysis showed a trend toward better overall survival in women treated with pertuzumab, trastuzumab, and docetaxel than in women treated with placebo, trastuzumab, and docetaxel, but the study had not gone on long enough for the survival data to be definitive.

On April 18, 2013, the study investigators reported an interim analysis of overall survival after an additional year of follow-up. They found that women treated with pertuzumab, tratsuzumab, and docetaxel lived longer than those in the two drug plus placebo group. At 30 months of follow-up, more than half of the women treated with pertuzumab were still alive, while the median overall survival for women in the placebo-containing group was 37.6 months. The risk of death at the time of the analysis was 34 percent lower for women in the pertuzumab group than for women in the placebo group. The findings were published in The Lancet Oncology.

The Food and Drug Administration approved pertuzumab on June 8, 2012, for use in combination with trastuzumab and docetaxel to treat women with HER2-positive metastatic breast cancer. This approval was based on the earlier progression-free survival results.

A San Fernando Valley native, Dr. Bruecker attended Pierce College then received his bachelors degree in Animal Science from the University of California at Davis.

He graduated from the University of California at Davis, School of Veterinary Medicine in 1983. After one year of general small animal practice in San Fernando, Dr. Bruecker completed an additional year of clinical internship at the West Los Angeles Veterinary Medical Group. He received his master of science degree at the completion of a three year surgical residency at Colorado State University and moved back to Ventura County in 1988 to establish specialty veterinary care. Dr. Bruecker is the Medical Director and Chief of Surgery at VMSG. He is also the Chief Medical Officer for Symphony, a veterinary specialty practice management group.

Dr. Bruecker provided regular surgical support for practices in the state of Hawaii from 1996 through 2011. He now serves as the Chief of Surgery for Veterinary Medical and Surgical Group- Orange County, as well.
A Board Certified Surgeon, Dr. Bruecker's primary clinical interests are spinal surgery, sports medicine/orthopedics (including arthroscopy, TPLO, TTA, and limb deformity correction), minimally invasive surgery (such as laparoscopy) and peri-operative pain management. He is well respected for his expertise in arthroscopy, limb deformity, disorders of the knee, fracture management and disorders of the spine. He has authored numerous articles and book chapters on Wobbler syndrome, treatment of intervertebral disk degeneration and spinal fracture management. He is an invited speaker throughout the United States, Latin America, South America, Europe, Asia and the South Pacific on a variety of topics in orthopedics (fracture management and arthroscopy), neurosurgery and pain management. He is an active participate in working groups on elbow dysplasia, shoulder injuries, advanced techniques in small animal arthroscopy and cranial cruciate ligament repair. Dr. Bruecker was the first to offer TPLO surgery, TTA surgery, cementless hip replacement, arthroscopy and laparoscopy to owners of pets in Ventura, Santa Barbara and San Luis Obispo Counties, as well as to the State of Hawaii. He has been an innovator in the development of new surgical techniques and orthopedic implants.

Dr. Bruecker is a past program chair of Neurosurgery for the American College of Veterinary Surgeons and a past program chair for the technician program for the American College of Veterinary Surgeons. He served as the orthopedics program director for 2004 and 2005 for the American College of Veterinary Surgeons. He was also program director for orthopedics, pain management and anesthesia for the 2006 American Veterinary Medical Association annual symposium. He is the program chair for the Association for Veterinary Orthopedic Research and Education (AVORE). He is a past Executive Board Member for Veterinary Orthopedics Society (2004-2007) and currently serving as the President-Elect (2013-2014).

Due largely to his commitment to education and training, Dr. Bruecker was selected as the Veterinarian of the Year for the State of California by the California Veterinary Medical Association in 2004.

Dr. Bruecker, his wife and sons are owner/operators of avocado and citrus farms in Ventura County.
