_Successful management of acute massive pulmonary embolism using suction catheter technique in a hemodynamically unstable patient._

Case Study:

Pulmonary Thromboembolism (PET) is a major Health condition with significant mortality and morbidity. PTE implies occlusion of pulmonary arterial circulation by the clot formed elsewhere usually in deep veins of the leg. Less than 5% of venous thrombosis occurs in other sites.

A 55 year-old female presented with complaints of breathlessness on exertion and giddiness with hypotension. She gave history of pain in chest since last two days. On evaluation and further investigations she was diagnosed with massive pulmonary embolism.
Investigations 2DEcho has shown RA, RV dilated with moderate pulmonary hypertension. Lower limb venous Doppler shown left vein Deep Vein Thrombosis.
CT Pulmonary Angiogram relieves massive pulmonary embolism.
Patient was initially Thrombolysed with Inj. Tenecteplase. Patient didn’t improve and on the way persistent hypotension and hypoxia. Pulmonary thrombectomy was planned. The patient was revived successfully with pulmonary thrombus suction with Indigo cat 8 suction catheter with controlled suction first time in Marathwada region.

Patient fine on follow up. This case highlights usual complication of DVT and excellent outcome of prompt management.

Alternate treatment was Surgical Pulmonary Thrombectomy in which mortality rate is high and success rate is very less.
