ACP’s mission is to convene safety-net providers, partners and other stakeholders to create a culturally competent, patient-centered, comprehensive and coordinated service care model to meet the needs of individuals and families participating in Medicaid. ACP is one of 25 Performing Provider Systems (PPS) in New York’s Delivery Reform Incentive Payment (DSRIP) program to transform the State’s Medicaid system. DSRIP’s goals include reducing avoidable hospitalizations and ensuring that patients and their families with Medicaid switch to a community-based care system that emphasizes preventative care over reliance on the emergency room.

Did you know there’s a syndrome leading to dry eyes and mouth that affects around 4 million people in the U.S.? 👀👄

It is called Sjögren’s syndrome and it is a chronic autoimmune disease that primarily targets moisture-producing glands, such as the ones that produce tears and saliva.💧 However, its impact can extend far beyond these symptoms, affecting various organs and systems within the body.

As of now, there is no cure for Sjögren’s syndrome, but it is manageable with treatments that focus more on alleviating symptoms and preventing complications.

Do you know what Hemochromatosis is? It's more common than you think.

Hemochromatosis is the opposite of anaemia. In fact, it is a condition where your body absorbs too much iron from the food you eat. But that is no good either, as that extra iron gets stored in your organs, especially the liver, heart, and pancreas, silently causing damage to your organs over time.

Symptoms are often silent or vague⚠️. They can include: fatigue, joint pain, abdominal pain, bronze or gray skin tone, and irregular periods or loss of libido.

The good news is that it can be detected with a simple blood test and it can be treated with a regular blood donation or phlebotomy.

Last month, we introduced you to the sneaky parasites that cause traveler’s tummy troubles🤢✈️

But parasites aren’t the only culprits... 👀 Bacteria are actually the most common cause of traveler’s diarrhea, and they act fast!

Parasites have a slower onset and can cause longer-lasting symptoms like bloating, fatigue, and may persist for weeks if untreated. While bacteria usually hit fast with stronger symptoms at the time, like diarrhea, cramps, or fever. However, they often resolve quickly.

Cystic Fibrosis (CF) is a genetic disorder that primarily affects the lungs, but also impacts other organs such as the pancreas, liver, kidneys, and intestine. It is caused by a mutation in the cystic fibrosis transmembrane conductance regulator (CFTR) gene, which is responsible for producing a protein that regulates the movement of salt and water in and out of cells.

When this protein is faulty, it leads to the production of thick, sticky mucus that can clog airways, trap bacteria, and lead to recurrent lung infections and progressive lung damage. This thick mucus also obstructs the pancreatic ducts, preventing digestive enzymes from reaching the intestines, which impairs nutrient absorption and can lead to malnutrition.

Raising awareness also plays a vital role in advancing scientific research towards a cure. By increasing public understanding, CF awareness initiatives contribute to a future where every person with cystic fibrosis can live a longer, healthier, and more fulfilling life.💙

Intestinal Methanogen Overgrowth (IMO) happens when some gut microbes produce too much methane gas, making your digestion slow and uncomfortable.

The good news is that IMO can be managed with specific antibiotics and diet changes, as the low FODMAP diet we’ve previously talked about.

Group B Strep is a common bacteria carried by about 1 in 4 pregnant women. However, it can be passed to the baby during childbirth, sometimes leading to severe infections like pneumonia, meningitis, or sepsis.

Because GBS often doesn’t cause any symptoms, many don’t realize they carry it, and it can be spread through sexual practices.

That’s why the CDC and the ACOG recommend that all pregnant women get screened for GBS between weeks 36 and 37 of pregnancy. This simple swab test helps identify if GBS is present, so antibiotics can be given during labor to protect the baby by dramatically reducing these risks.

Melanoma is one of the most common cancers worldwide.. This , let's shed light on the importance of sun safety and regular skin checks.

☀️ Sun safety first: Wear sunscreen with SPF 30 or higher, seek shade, and wear a hat when under the sun!
🔍 Check yourself up: Keep an eye out for any changes in moles, freckles, or new spots. Early detection is always key!

Happy Independence Day!🇺🇸 Today, we celebrate the spirit of freedom, unity, and the red, white & blue! ❤️🤍💙 Whether you're enjoying a BBQ, watching a parade, or lighting up the sky, remember to keep it safe.

Here are a few simple tips:
🎆 Keep a good distance: Enjoy fireworks from at least 25–50 feet away.
🎆 Never relight fireworks that don’t go off: Wait 20 minutes, and then soak them in water.
🎆 Light one firework at a time: And move back quickly.
🎆 Have some water nearby: Have a bucket ready just in case.

Sarcoma is a rare and often overlooked type of cancer that develops in the bones and soft tissues, like muscles, fat, nerves, and blood vessels. It remains one of the least understood forms of cancer making up just 1% of adult cases but around 15% of childhood cancers.

Because sarcoma often shows few early symptoms and can appear almost anywhere in the body, diagnosis can be delayed. Most cases have no clear cause, and unlike some cancers, there’s little that can be done to prevent it.

Despite the challenges, early detection and advances in treatment, including surgery, radiation, and chemotherapy, offer hope. Many people go on to live full, healthy lives after treatment.

This July, we stand with patients, survivors, and families. Let’s raise awareness, support research, and ensure no one fighting sarcoma feels forgotten.

Congenital Diaphragmatic Hernia (CDH) is a rare birth defect that occurs when the diaphragm, the muscle that separates the chest cavity from the abdominal cavity, does not fully form during fetal development. This incomplete formation allows abdominal organs such as the stomach, intestines, and liver to move into the chest cavity, crowding the lungs and preventing them from developing properly.

The severity of CDH can vary, but it often leads to underdeveloped lungs (pulmonary hypoplasia) and high blood pressure in the lungs (pulmonary hypertension), both of which can pose serious challenges for newborns after birth.

Treatment for CDH typically involves surgery to reposition the abdominal organs and repair the diaphragm, often within the first few days of life. However, because of the associated lung and heart complications, newborns with CDH often require intensive medical support, including breathing assistance, medications, and sometimes extracorporeal membrane oxygenation (ECMO) to support heart and lung function.

Long-term outcomes depend on the severity of the condition and the effectiveness of early interventions. Advances in prenatal diagnosis and neonatal care have improved survival rates and outcomes for many infants born with CDH.

Would you know what to do if someone's having a stroke? 🚨

Today, we want to underscore the urgency of recognizing stroke symptoms and acting swiftly, emphasizing that timely medical intervention can significantly improve outcomes and reduce long-term disabilities.

If you observe any of these signs, even if they are mild or disappear, it's crucial to call 911. Do not hesitate or delay. Calling emergency services immediately ensures that the person receives prompt medical attention and can be transported to the nearest stroke center.

Irritable Bowel Syndrome (IBS) is a common gastrointestinal disorder that affects the large intestine, leading to a range of symptoms including abdominal pain, bloating, gas, diarrhea, and constipation.

Although the exact cause of IBS is not fully understood, it is believed to involve a combination of gut-brain communication issues, heightened sensitivity of the intestines, and changes in gut motility. Triggers can vary from person to person, including stress, certain foods, hormonal changes, and infections.

IBS is a chronic condition, but it does not cause permanent damage to the intestines or increase the risk of more serious diseases.

Managing IBS often requires a multifaceted approach that includes dietary changes, stress management, and medication. Common dietary strategies include the low FODMAP diet, which we talked about in a recent post. Some people find relief through probiotics or fiber supplements, while others benefit from cognitive behavioral therapy or relaxation techniques to reduce stress.

Although IBS can significantly impact quality of life, it can often be managed effectively with the right combination of lifestyle adjustments and medical support.
