This year's last elective surgery...

This year was professionally a fulfilling one. Had my fill of aesthetic procedures, pediatric plastic surgery, trauma reconstruction, maxillofacial injuries, burns and a couple of microsurgical procedures...

However, one thing missing from my spectrum of surgeries was resecting some rare tumor of sizeable size....to be neatly excised!!!

Was thinking of the same a couple of weeks ago that haven't done this since a long time.. And here came in a young adult with this large tumor of the leg excised a few years ago elsewhere with recurrence. Incisional biopsy suggested a rare tumor called as Collagenous fibroma and we did his tumor excision in toto today!

A perfect way to wind up electives for the year 😊😇🤞🧿🧿🧿!

I would like to thank all my colleagues, staff and friends🙏🙏🙏 who have been placing their immense trust in my skills and sending me their patients year after year👬👭👫... My patients who sent in their relatives and friends to me for surgeries... And my old patients who came in for their second unrelated surgeries to me again...

I assure you that your trust in me and my skills shall always remain the same and each of your patients would get the best of me🧿🧿🧿🤞🤞🤞! Thank you all from the bottom of my heart for giving me all the amazing opportunities to help heal!

A woman friend and colleague's house warming of a home that she bought on her own with her own hard earned money... And a lot of thoughts after that...And there comes a poem after long titled...


"स्त्री का घर "

मायके से एक बार बाहर निकली स्त्री,
का कोई घर नही होता...
जिस आगंन से पाल पोसकर बडी होकर,
वह एक बार पढ़ाई करने निकल जाती है...
फिर वह कभी घर नही आती...

सोचकर तो यह जाती है,
कि पढ लिखकर, कामयाब होकर,
एक दिन माता पिता की सेवा करूगी,
और भाई बहनो की प्रेरणास्रोत बनकर लौटूंगी...
पर फिर वह कभी घर नही आती...

आती है तो क्षण भर के लिए...
किसी के घर की वधू बनकर,
एक आगंतुक परिवार के घर,
विदा होकर जाने के लिये...

मन मे सपने संजोए कि,
शायद आखिरकार अब मुझे
वह घर मिल गया,
जिसे मैं अपना कह पाऊँगी...

पर वहां अधिकतर स्त्रीयों को मिलती है,
केवल जिम्मेदारियाँ, झिङकियां और तिरस्कार,
जिस संतान को वह जनती है,
वह तो उस घर का हो जाता है,
पर स्त्री कदाचित नही...

उसके हिस्से आती है टूटी फूटी तारीफें,
वो भी कभी कभी,
अपने सपनों का गला घोटकर,
दूसरों के सपने रौशन करने के लिये...

फिर एक दिन जैसे किसी दुस्वप्न से उठकर,
वह चुपचाप निकल जाती है,
सुख शांति और
स्वयं को ढूंढने...

और फिर वह स्वयं की मेहनत से बनाती है एक घरौंदा,
जिसके दर दरवाजे उसका प्रेम से उसका स्वागत करते हैं,
जहां उसके अट्टहास, स्वाभिमान और सुख- शांति को कोई दृष्टि नही लगाता,
जहां उसे कोई नही कहता कि यह घर तुम्हारा नही...

When someone asks what is your favourite instrument and you can't pinpoint one......

For you have used everything from Hudson's brace, to Thudicum's nasal speculum to corneal shields to Hegar's dilators to mastoid retractors to chisels, hammers, osteotomes, bone nibblers to gigli saw to Doyen's and Deaver's abdominal retractors....besides your favourites Humby's knife, loupes and microsurgical set...

So all you do is just smile 😊... And say..."Any instrument that can be used from head to toe...that can help ease my particular surgery of that day is my favourite!!!"

Being a plastic surgeon and treating a massive variety of conditions from head to toe is a vibe unlike any other!!!😎😅!

Happy to be serving as one!

Ear lobe reduction surgery.

This lady presented to me a week ago with complaints of large sized ear lobules and poorly done ear lobe suturing done by a local doctor which was making her unhappy on a daily basis.

She said that she chose him because he was the closest doctor to her home. When I asked her if he was a , she said NO.

Do you choose your especially a what are the criterias that you keep in mind? I hope distance and the cost of the surgery aren't the most important of them.

The most important criterias that should be seen are:

1) Educational qualifications and even the Institutes from where they have trained.
2) Experience
3) Areas of special expertise
4) Added fellowships/trainings pertaining to the treatments they offer.

While distance from your home is an important consideration, it comes after the above criterion.

Unbelievably lower cost of surgery may appear lucrative, they are often due to inexperience or absence of actual qualifications or due to compromised operating conditions all of which may pose a risk for bad results and sometimes even life threatening conditions.

This lady came today for her follow up and was extremely happy with the fine results that she sees now. She said, "After a very long time I'm happy now looking at my earlobules. They were so ugly after previous surgery and every person who used to see me used to ask what happened to your ear!" which was causing her major psychological damage.

And we as always, were happy to help 😊!

A page from an old register... And tons of memories of good old college days...

In preparation of our 25 year alumni meet...friends are coming out with all that they have with them of the beautiful days bygone...when we were just young girls and boys...carefree and not exposed to the ways of life!!!

This snippet from our attendance register shared by my friend Nazia Jamal who was roll no 32 a while ago on our alumni group, took me back in time... The dissection tables, the lecture theatres and our names being called for attendance...Some raising hands for themselves and some putting in proxy attendances...

All that seems like a fairytale now...A dreamy wonderland of which we all were part in all our innocence...

And every single picture shared, every single snippet or memory shared is evoking major nostalgia...

There are friends...And there are special friends...the ones who truly try their best to stand by you!

One such amazing friend who is an amazing ophthalmologist and an avid reader, had her birthday today!

All of us were jam packed with clinical work today but how can we miss to surprise special friends on their birthday. So postponing the big party to another leisurely day and just catching up for a quick coffee and some conversations!

And when I gifted her a book signed by Shashi Tharoor...She exclaimed..."Is this all you bought? I thought you would gift me the author himself!!"

And over conversations we realized that we all were somehwhat gloomy last evening, each one for different reasons...but our tiny meetup rejuvenated us all back to normal...

And we came up with a slogan while going back home..."The only sureshot way to turn gloom to bloom is to catch up with friends who know you well!!"

A beautiful half an hour amidst being swamped with work!

Word of mouth...

A 60 year elderly woman,
from Villuppuram, Tamil Nadu,
Comes to me in Bangalore today,
Leaving the nearby plastic surgeons of closer city Chennai, and Bangalore city behind.

Not because I'm better than all of them,
But because her daughter-in-law,
My former colleague,
says she trusts me and my skills the most, for her.

While the world runs on digital marketing and paid leads...
The truth is that word of mouth for a doctor,
is the most beautiful of all recommendations,
that ever was and that ever will be...

Else who crosses states?,
And cities?,
and borders
Just to see a plastic surgeon!!?

I tend to her, with,
Perhaps a bit more gently than I usually do,
For she had crossed hundreds of kilometers in her faith for me.

And as she leaves with gratitude after the surgery...I reflect...
What a huge honor..., A huge responsibility...,
And a humbling experience, word of mouth is!

Decorum in OT and workplaces...

It was a routine day and I was engrossed in my procedure as always. Suddenly I hear one of my female staff getting upset and angry while another of my male staff was speaking something and giggling constantly. Both are young people around 22-23 years of age.

Usually I'm deeply entrenched in my work that literally keeps me deaf to any unwanted noises but today was different and this tussle made me look at both of them. Since they were both speaking in Kannada, I didnt get much of what they were speaking but since I saw that girl look uncomfortable, I asked her what happened. She told me that the boy was cracking some jokes at her expense which she was not liking.

I had to immediately intervene and tell that boy to stop the joke. He said it was just a minor teasing and nothing else.

However, one needs to understand boundaries and what may be acceptable to some may be completely unacceptable to the other person. And that must always be respected, more so at the workplace. I'm no one to do moral policing but such conversations must stop right when one sees that the person at opposite end is not liking it.

The girl looked reasonably happy after my intervention and the boy was silent after that.

I didn't want to be a judge, but being in the position of the team leader had to stop it!

Are you mindful of what and how you speak at your workplace especially with colleagues of opposite gender? If not, you must learn to be!! And if someone is uncomfortable with your jokes, please keep them to yourself!

About last night!

We had the last bimonthly meet of Plastic surgeons of Karnataka of year 2025. Slight chill in the weather making for a pleasant evening. A bit of learning, conversations over dinner and a meet up with friends and colleagues...

Terror 😱 in the OT!

A couple of days ago, one of my OT assistants gently muttered, "Ma'am you are the surgeon we all are scared of 😖, the most!!"

While his comment took me by surprise, I asked him, "Why do you feel so?"

He said,"You scold us at every little mistake and expect too much of perfection."

I laugh,"Is it? But my patients come to me for the same reason. They want perfection from me. If you prepare the OT better, there will be no need to fear my wrath! 😅"

Here are the ways OT staff can keep their happy:

1) Post the case on time, unless there comes in a last minute emergency.
2) Ask for the sutures and the special instruments needed, well in advance, preferably a day before.
3) Take care that patient reaches only after proper part preparation, receives the presurgical antibiotic shot and all other pre-op intsructions are duly followed.
4) Ensure a good bore canula in a good lumen vein.
5) Never put canula in the same limb as the operative site.
6) Never put patient tags in the same limb that the surgeon wants to operate.
7) Check that the patient details are entered correctly on the board before surgery starts.
8) Wash before the surgeon does and be vigilant of the steps of surgery. You should be able to know what instrument the surgeon will need next and hand them accordingly.
9) A bit of reading about the concerned condition before coming for case carries bonus points.
10) Maintain a cheerful, enthusiastic and well groomed disposition. Being a surgeon is already a serious and demanding job. The last thing we want is dull, unenthusiastic staff in the OT.
11) Diligently follow the post op instructions till the patient is in the recovery room and give a proper handover to ward staff when you shift patients.

And last but not the least, never hesitate to ask questions ❓️, as many as you may have, even the ones you think may be foolish🫣! We would love to answer them 😊and to help you learn a bit more about a surgery that you wash for!

The only place I possibly could be during this season is the this being an event that literally makes me think further and outgrow myself and my thought processes each time I am here.

Had lovely time at the sessions discussing languages, mythology, poetry, what do men fear, breaking news, how to keep oneself sane in this age of digital media, world of crime thrillers and last but not the least Shashi Tharoor's amazing session about words which though the last one had the venue packed by more than 200% 👫👭👭🤩😁of its capacity.

And meeting some of my friends whom I have made over all these years like an annual ritual was a reminder of how love for arts and expecially literature has drawn us to each other...

Had my fangirl moments with Jeet Thayil, an amazing author and poet who still writes the accurate traditional English as he was sitting next to me in one of the sessions.

And then another fangirl moment with Dr Shashi Tharoor. The queue for getting his books signed was almost 500-600 people long.. and I must have been the 400th in that line... It was told that he would just be signing books but not able to write personally considering the huge queue...But when I went to him and said to him, "Sir I have 3 books to be signed. I would really appreciate if you could atleast sign one of them for me with my name on it", he said, "Dr Neha I haven't done that for anyone else but since you have requested me personally, I cannot say No. But don't tell anyone, they may get upset.." and with a smile he signed one of them with my name and a gentle but sound advice..."Keep reading"

Didn't realise how the day went and when night set in... until I saw the beautiful moon 🌕 by the side of a coconut tree🌴... Perfect way to end the day for someone who loves literature 📚😇!!

The beautiful venue of Freedom Park with its rich history made for a mini adventure, exploring the former central prison was an adventure of sorts...
