Honored to be part of the 27th TN AOI State ENT Conference, Alaigal 2025, at Ocean Spray Resorts and Pondicherry Institute of Medical Sciences. I was honor to deliver guest lectures on Management of Skull Base Paragangliomas : State of the art and Temporal bone malignancies and also demonstrate live cadaver dissection on lateral skull base dissection. My sincere thanks to Dr. Mohan Kameswaran (MERF), Dr. Vijay Krishnan (MERF), Dr. Raghunandhan Sampath, and Dr. Karthikeyan for the opportunity and warm hospitality. Case of the Week Series

A 58-year-old diabetic male presented with a 8-month history of severe nocturnal ear ache and profuse ear discharge unresponsive to medical treatment. He also developed facial weakness during the course of his illness. On examination, he had an edematous and inflammed external auditory canal filled with granulations. Neurological evaluation revealed a facial palsy of House Brackmann grade 5. He also had palatal palsy and a reduced gag reflex.

High-resolution CT and Contrast enhanced MRI of the temporal bone and Skull Base revealed erosions of left half of the clivus, left petrous bone, left lateral wall of sphenoid sinus and left occipital condyle. Soft tissue inflammation was seen involving the prevertebral space, left parapharyngeal space and carotid space. The disease was found to infiltrate into the jugular fossa and causing narrowing of the petrous portion of left ICA. An ovoid focus seen posteromedial to the ICA, measuring 7.5 x 4 mm and showing mild enhancement on post contrast sequences was likely to be a mycotic aneurysm.

He subsequently underwent surgical intervention in the form of Infratemporal Fossa Approach Type B. During gentle manipulation of the mycotic aneurysm, there was a spurter which was controlled by surgicel pressure. Muscle and surgicel was used to cover the mycotic aneurysm and there were no further bleeding episodes. Disease clearance was then completed. Aerobic cultures revealed Pseudomonas aeruginosa and patient was given Meropenem for 4 weeks postoperatively. He was then started on oral levlofloxacin for 3 months. He has been on regular follow-up and is doing well.

From relentless ear pain to cranial nerve palsies and a rupturing ICA aneurysm — Skull Base Osteomyelitis fought hard, but surgery and targeted antimicrobial therapy won harder!!
