49 Y Old Male, Sales executive, 20 Yrs for two-wheeler driving daily around 150 KM.
5 Y H/O frequent back pain, which he applied pain balm and managed.
Three months back, he developed foot and toe weakness.
The Doctor Told MRI and then advised Surgery because a toe drop is a sign of a red flag.
The patient denied surgery and took Physiotherapy elsewhere, IFT, and Exs. Though he thinks he is better, the toe drop is yet to improve.
He met me last week for a second opinion. I told him to wait and watch since the golden period for Surgery is over, and pain is not a major complaint. Taught him spine hygiene strategies. He will meet me after one month.

Message:
1. Some chronic Back pain can lead to cauda equina syndromes(CES). Pain leading to Paralysis
2. Once CES establishes immediate surgery can give faster improvements
3. Once delayed surgery, the timeline of improvement can't be asserted
4. Some CES will anyway improve with time regardless of the treatment
5. Some CES will worsen with time if the underlying disc bulge is progressively compressing the nerve.
6. Pain is not directly proportional to weakness. Some can have only weakness without pain. It is called Radiculopathy
7. Younger people with CES suffer more with Older people due to high neuroinflammation
8. Physio can play only a limited role in CES. Just activity modification and posture care.
9. Strength Training won't help unless the compressed nerve is relieved of the pressure.
10. CES is Preventable only if the pain is addressed during the initial day. Usually pain will be felt decades before CES develops. Avoid Pushing through the pain.
