When a patient says, “Wont a dental implant last longer?”

Sometimes the answer is ‘yes’ and sometimes ‘no’

It’s ever so important that patients understand the benefits of a periodontal ligament that is retained by having root canal treatment rather than extraction!!
This is so underestimated by patients and many many dentists.

Retaining a tooth by having root canal treatment means retaining the periodontal ligament.

Why is this important
- functioning on a tooth stimulates the ligament which then causes forces in the alveolar bone so that it remains in place.

- we’ve all seen full denture patients with resorbed ridges — this is a result of plastic plates compressing bone - compression causes Resorption- in the same way that wearing a ring on your finger also results in bone resorption over time !

The tension that functioning on a tooth creates maintains bone!
As a result of this the aesthetics remain unchanged.

- implants have no periodontal ligament and although no compression occurs bone is lost in time around implants. This is expected and In the long term leads to aesthetic problems and sometimes peri-implantitis.

So if you can keep the patient one step away from needing that implant by saving their tooth
