YOUR SUNSCREEN!

It is a little disappointing, that even in this day & age, with all the information available at our fingertips, most people still think of sunscreen as merely for sun protection.

Light skinned people wrinkle early.
People of colour age in the form of pigmentation, which starts from the 2nd or 3rd decade of life. Wrinkles appear later.

If you start using a high-grade, broad-spectrum sunscreen , EVERY day, from your 20s – In the long run, this is your BEST defence against pigmentation, wrinkles and open pores as well.
Making sunscreens, hands down, the least expensive and still, the most effective anti-ageing cream.

People are willing to spend a lot of money buying expensive “skin lifting” or “wrinkle-removal” serums & creams. Fact is, these creams do almost NOTHING to tighten your skin, “seal up” your pores or eliminate wrinkles that have already formed. And yet, the humble sunscreen either gets ignored, or you will reach out for any random low-grade sunscreen to fulfil your skin care needs.

IN TODAY'S POST ON "BEAUTY PRODUCTS YOU CAN LIVE WITHOUT" .. I will be talking about certain beauty supplements.

Nutritional support is no doubt an essential part of skin, hair and nail care, as well as an important part of treating various diseases of the skin and hair.
There is no arguing that.

Which is why, a lot of companies – pharmaceutical, cosmetic as well as home-grown, have exploited the layman’s general belief that diet and nutrition is a fix for any and every skin or beauty related concern.

A lot of patients are excited about “beauty gummies”. Gummies are nutrient poor and rich in sugar and gelation, that you do not need. Consult your Physician for advice on supplements – based on your age, general medical condition and your specific needs.

Collagen powder is overrated as well. Bovine collagen may have benefits if you are an athlete. But the benefits of marine collagen are still under scrutiny and not proven. Take Choline Orthosilic acid supplements instead – these are proven to strength hair and nails and boost natural collagen production in your skin.

Most people perhaps take Glutathione for all the wrong reasons as well. Glutathione does not work on serious pigmentary disorders. When taken in oral or I/V form, it can hydrate and brighten your skin. It works best in quickly clearing up a recent tan. However it cannot lighten your original skin tone.

Don’t even get me started on intravenous “beauty” drips and elixirs that promise to fix your hair fall or make you any younger or fitter!

Lastly, any benefits of collagen powder or glutathione can only be evident after several months of consistent use. Also, high grade collagen powder and glutathione are fairly expensive – So if you are on a tight budget, you can easily skip these supplements. – Dr Sunaina Hameed, MD

Makeup & Sunscreen wipes!
Make up is best removed through double cleansing. Put a few drops of your make-up remover on a cotton pad and wipe gently to remove make-up. Then use your facewash to rinse your face. Toners do not effectively remove make up. Toners and make up removers are not to be used as face washes – or vice versa.
Sunscreens are least effective when used in the form of a mist or stick or wipe. Use a cream, lotion or gel based sunscreen instead – Dr Sunaina Hameed, MD

Face mists are generally a watered down version of toners.
These are generally soothing agents containing spring water, rose water or such.

Sure, if your skin tends to be flushed and sensitive and if you spend a lot of time in the sun, these could provide some instant soothing. However, if your budget is limited, you can easily skip face mists from your skin care routine and you’ll really not miss a thing – Dr Sunaina Hameed, MD

MORE ON "BEAUTY PRODUCTS THAT YOU DO NOT NEED" ..
CLEANSING & SCRUBBING DEVICES !

Hand held facial cleansing and scrubbing brushes are not only unnecessary, but can be downright damaging for facial skin! They were never really meant for daily use – and yet it is used way too frequently, or falsely advertised as safe for daily use.

If you are looking for thorough removal of make-up and/or oil debris, I would prefer a double cleansing technique – which is safe on sensitive and acne-prone skin as well. Use an oil based or micellar water containing make up cleanser, followed by your regular facewash. Using your FINGERS!

Cleansing & Scrubbing devices do not shrink pores or “scrub away” fine lines and pigmentation. Blackheads and whiteheads usually only respond to anti-acne topicals like Adapelene or Tretinoin. They cannot be scrubbed off using such tools. In fact, if you use these devices while failing to moisturize, they might actually make your skin look more dull, dry and rough – Dr Sunaina Hameed, MD

Is there more SKIN CARE THAT I DO NOT REALLY NEED?
Yep. Creams for strawberry skin.

Keratosis Pilaris, popularly known as “strawberry skin” is a skin type rather than a skin ailment. Humidity and moisturizing certainly improves the appearance of this condition. Exfoliating agents like Salicylic acid, Lactic acid and Tretinoin along with moisturizers containing ammonium lactate or urea can soften the skin and lighten the pigmentation. However, this condition is very hard to eliminate completely. And it always, always tends to recur.

This condition is often mistaken for ingrown hair. Ingrown hair responds very well to Laser hair removal treatments. Keratosis does NOT respond to laser treatment. So you will need to consult a Dermatologist for a definitive diagnosis, before you sign up for any laser treatments.

If you’re prepping for a major event like your wedding, consult your Dermatologist 5-6 months before the event. Skin care will be prescribed along with monthly chemical peels, that can significantly improve the appearance of your skin, in time for your event. In our ongoing series titled "SKIN CARE THAT YOU DO NOT NEED", its time to talk about Cellulite removal creams.

Or for that matter, any fat reduction, skin tightening, breast lifting, b**b enhancing, breast reducing and other such topical formulations.

Cellulite is hormonally driven, which is why we almost never see cellulite on men and children. A topical application is highly unlikely to provide any real or lasting benefit in “treating” cellulite. Bronzers and body make up do a better job of “hiding” cellulite – until you can get your diet and exercise regime in order! In continuation of our series on SKIN CARE THAT YOU DO NOT NEED - Today I write about SCRUBS.

The era of physical scrubs is on a popularity decline. And with good reason. We still believe in exfoliation. But chemical exfoliation using alpha or beta hydroxy acids (to name a few) is the safer and more effective way to exfoliate.

Physical exfoliation using a scrub was never suitable if you have active acne on the face. It does not get rid of blackheads and whiteheads. It does not get rid of open pores. It does not make cellulite go away. It does not “drain toxins” from your skin. In fact, over enthusiastic scrubbing can make acne worse and lead to pigmentation.

If you DO need to do physical exfoliation, say for example, if your skin cannot tolerate chemical exfoliating agents, then use it only once a week. And do not use shell based exfoliants (walnut or apricot shell, for example), as they cause microtears in the skin. Sugar scrubs are too harsh as well. These microtears only worsen your skin, if you are not using the right skin care and the appropriate precautions.

I am starting a new series of posts on the topic - SKIN CARE PRODUCTS YOU DO NOT NEED.
This is inspired by my daily conversations with my patients. Lately it seems like every nook and cranny as an ëntrepreneur who is concocting their own skin care range. And leaning heavily on online promotions and paid blogger/ vlogger/ mini celebrity endorsements. Heck, even celebrities have joined the bandwagon and are "creating" their own line of skin and hair care.
Efficacy and quality is a discussion for another time.
But firstly - Do we need ALL of these products?
Take TONERS for example.
Toners are not necessary for skin health and has no special benefit in any skin condition.
They are promoted in various lines claiming various benefits from pore tightening, acne reduction, redness reduction, deep cleansing or hydrating toners.
In most cases, they just irritate your skin, leaving it less tolerant to skin care with active ingredients. In fact, most of these properties can be found in a good quality moisturizer and anti-acne topicals, which ACTUALLY work!
Toner may only have some benefit for those who need to use a lot of make up on a regular basis. However, we now have access to a wide range of cleansing oils, make up removing balms and micellar water for make-up removal. SKIN CARE GIFTING IDEAS ..

Every now and then, I will receive a patient who has suffered an adverse reaction to some skin care product. While we may be cautious or even tight-fisted while purchasing skin care for ourselves, we throw all caution to wind when we receive some fancy beauty product as a gift. The gifter will also suffers tremendous guilt if their presents cause allergies and suffering. The wrong gift can also lead to hurt feelings!

Effective skin care requires thorough knowledge of a person’s skin type and skin issues. In general, you probably should’nt give skincare as a gift.
But here are some simple gifting ideas that will not wreck skin havoc and will be well-loved –

1) Hand & Foot cream – Hands get washed multiple times a day and are exposed to multiple contact allergens. Feet are the most neglected part of our anatomy. No wonder that our hands and feet are most likely to give away our age! Gift a loved on with a good hand & foot cream, to encourage them to begin caring.
2) Sheet Masks – Safe, hydrating, inexpensive and come in a variety of choices.
3) Micellar Water – Great as a gentle toner and excellent make-up removal product as well. This is light-weight, generally unscented and suitable for all skin types.
4) A luxury nail care kit containing cuticle oil, nail buffer, nail cream and a strengthening nail paint.
5) A leave-on hair conditioning serum – Something light weight like Argan oil. Leave a note suggesting to use it only along the length of the hair, and not on the scalp.
6) Unscented or mildly scented cocoa butter containing body creams.
7) A light weight pre-shower bath oil.
8 ) An electronic foot file/ callus remover.
9) A 10% Vitamin C serum or Hyaluronic Acid serum for the face.
10) A CoEnzyme 10 containing unscented anti-ageing moisturizer.
11) Bronzers and leg make-up

Here is a list of DO-NOT-GIFT items –
1) Scented toiletries
2) Body scrubs and loofahs
3) Physical or Chemical Exfoliators – NO acids (Salicylic/ Glycolic/ Lactic), NO Retinol.
4) Acne-related skin care – THIS is the Dermatologist’s domain. Acne products can be notoriously drying and irritant.
5) Scented cosmetics, skin care and/or toileteries

Do bear in mind that if your loved one is suffering from acne, pigmentation and/or facial allergies, they need to consult a qualified Dermatologist. Buying stuff over-the-counter based on reviews and hearsay is not the smart way to show you care – Dr Sunaina Hameed, MD

WHAT IS BAKUCHIOL?

This molecule was first isolated from Psoralea corylifolia seed in 1966. However, it has gained popularity in dermatological and cosmetic circles in the last few years – as the herbal alternative to Retinol.

While Dermatologists swear by Retinol, it does not work for everyone. For those who have experienced severe irritation and sensitivity to Retinol, Bakuchiol would be a worthwhile alternative anti-ageing molecule to consider. Hence, it is suitable for all skin types.

It helps in improving our skin by increasing skin turnover, acting as a chemical exfoliant, hence improving tone and texture, and reducing the appearance of fine lines, wrinkles and open pores. It is gentler than Retinol, and can be used every day or even twice a day. It is an antioxidant that can stimulate collagen formation. It has anti-inflammatory and anti-bacterial properties as well, which renders it effective in acne control.

However, since it is a chemical exfoliator, it will be imperative to add sunscreen and moisturizer to your skin care regime. It works well with hydrators like hyaluronic acid, polyhydroxyacids and squalene. It is best not combined with Glycolic acid and Vitamin C.

It is sold in serums and creams and is occasionally found as an ingredient in facial cleansers as well. Use a formulation with concentration between 0.5-2% for best results. If your suffer from eczema, atopy or rosacea, check with your Dermatologist before use – Dr Sunaina Hameed, MD

WHO IS A DERMATOLOGIST?

A qualified Dermatologist is a Doctor who has completed 5.5-6 years of undergraduate medical training followed by 2-5 years of postgraduate medical training in a medical college/ university.
A Dermatologist is trained to treat conditions and concerns related to skin, scalp, hair, nails, external ge****ls and the oral cavity (not including teeth and gums). This includes all age brackets, from cradle to grave.
Since a Dermatologist has received intensive training to treat conditions of the scalp and hair, all Dermatologists are Trichologists by default. This also means all Dermatologists are competent Pediatric Dermatologists as well.

Some of us may take up additional Fellowships in Laser Medicine, Dermatosurgery, Hair Transplantation etc, to add to these specialized services to our practice.

Homeopaths and Ayurvedic Doctors can also competently treat skin and hair related conditions with the medications that they are trained to use, from their own branch of medicine. However, they have not received adequate training or nationally/ internationally recognized qualifications or certifications to competently treat skin and hair conditions with Allopathy medications and modern Aesthetic treatments like chemical peels, lasers, injectables, to name a few.

Your Dentist, Hairdresser, Makeup artists and random medical/ non-medical individuals, claiming to be Dermatologists/ Trichologists after taking a crash course at unrecognized institutes, are NOT Dermatologists.

Your Dermatologist is not a Hairdresser and salon Aesthetician, so no, they do not perform hair color/ hair smoothening, pedicures, party make-up etc. You may mostly find Dermatologists talking about cosmetic problems and their remedies on their social media handles . Many of us may not even wear our white coats in our private practice. We may mostly speak about cosmetic concerns because these issues grab maximum eyeballs, and we can safely speak on these issues in broad terms. However, a Dermatologist does MUCH MORE than fix mere aesthetic related concerns.
