A typical glasses prescription includes your name, a unique patient ID, and several important measurements. These usually cover Visual Acuity (VA), Spherical Power (SPH) which corrects nearsightedness or farsightedness, Cylindrical Power (CYL) which corrects astigmatism, and the Axis, which indicates the lens orientation for astigmatism correction. Prescriptions often include separate readings for distance vision and near vision, shown in two distinct rows.
