Eye Pain That Won’t Go Away? The Hidden Reason Behind It | Expert Opinion

Eye pain that just won’t go away—even when tests look normal—can be confusing and frustrating. What causes this persistent discomfort?
Many people experience chronic eye pain without visible signs or symptoms, and in such cases, regular eye drops or treatments may not provide relief. So what could be the reason behind this?
In this video, Prof. Dr. Rohit Shetty, Refractive Surgeon and Translational Scientist, Director, Narayana Nethralaya, Bengaluru, explains the science behind persistent eye pain and how it can be treated.
One possible cause is chronic inflammation in the cornea, which can lead to peripheral sensitization—a condition where the eye tissues become extremely sensitive to pain. A common trigger for this is long-standing, untreated dry eyes.
If this inflammation continues for a long time without proper treatment, it can progress to central sensitization. In this stage, the neurons in the brain become hyper-excitable, making a person hypersensitive to pain. Here, the source of pain is no longer just the eye—but also the brain.
Because this type of pain involves both ocular inflammation and neurological factors, treatment often requires a multidisciplinary approach, involving ophthalmologists, neurologists, psychologists, and behavioural therapists. Addressing the neurological component is crucial for effective relief.

Glaucoma is usually managed with regular eye drops to control eye pressure. But for some people, using drops every day can be difficult, or the pressure may not be controlled well enough.

In such cases, surgical options can help manage glaucoma. One of the newer options is MIGS (Minimally Invasive Glaucoma Surgery). It uses very small incisions to lower eye pressure and usually allows faster recovery. In some patients, it can also be done along with cataract surgery.
