40 Y M, 11 Endo failure, large periapical lesion, root resorption, mobility.. what will you do?

I’d posed a question yesterday with this case. Here’s what I did. Glocker style ridge preservation for 11, Apicoectomy and retrograde MTA on 12, implant placement 6 months later on 11. Did a core biopsy- beautiful regeneration and I mean regeneration, with nearly complete turnover of the carbonated hydroxyapatite. 3 months later at restorative stage ISQ was 78. Final restorations- layered Zirconia

Large periapical critical size defect, draining sinus in the attached gingiva between 12&13, incisor (11)marked for extraction , super thin facial plates… what would you do?

This is an interesting one for a variety of reasons- The size of the defect, the open healing protocol, the tissue master concept and the difficulty of working on a very apprehensive patient in COVID times

Here's a tough case. Tough cases are not only the ones that are demanding surgically and restoratively. Some patients can be super demanding, which just makes the degree of difficulty much more. This one was tough in all respects.

Reviving this page with a new identity. Here's something interesting to begin- Guided Block harvest and positioning inspired by Luca De Stavola for a middle aged lady with Sjogren's syndrome.The turnover of both the autologous bone as well as the Porcine Carbonated Hydroxyapatite were excellent both radiographically and clinically, however, there was a deviation from the original plan at implant placement, with final position getting deeper than intended.
