🧠 New Insights Into Migraine: How Brain Fluid Triggers Pain
A Science article (Vol. 385, Issue 6704, 4 July 2024) offers a groundbreaking perspective on migraine. Traditionally, migraine pain is believed to start after a brain event called cortical spreading depression (CSD), often associated with visual aura, which activates pain-sensing nerves (nociceptors).
Previously, scientists thought that cerebrospinal fluid (CSF) couldn’t directly affect the trigeminal ganglia—a key cluster of nerve cells involved in migraine—because these ganglia were believed to lie outside the blood-brain barrier (BBB).
But now, Rasmussen et al. have shown in a mouse model that after a CSD event, signals carried by CSF can directly reach and activate the trigeminal ganglia, bypassing the usual pain pathways in the meninges. Even more surprising, their findings suggest that these ganglia are actually inside the BBB, challenging long-held assumptions.
This discovery opens a new path for migraine treatment by targeting how CSF communicates with nerve cells during the early stages of a migraine.
