We successfully managed a series of challenging cases -

1. A 2 year old child swallowed a silicon tip of a dart gun. The tip was stuck on the posterior wall of Upper Oesophagus and removed after gentle manipulation.

2. A 1 year old child got a piece of Supari stuck in his Left Main Bronchus, blocking air entry to left Lung. It was successfully removed using Optical forceps.

3. A 45 Year old lady had a Fish bone stuck in her throat. She was seen by 3 ENTs and counseled that there was nothing and it will stop hurting soon. The bone was visualized by flexible endoscopy and removed under Local Anaesthesia.

4. A 5 month old baby with history of Intubation at age 1 month presented with progressive breathing difficulty. Significant post-intubation subglottic stenosis was found which was treated Trans-orally using Laser and Balloon Dilatation. The baby is now 8 months old, has no breathing difficulty, and has caught up with weight according to age.

5. 6 month Old baby with significant failure to thrive (weight 2.5kg) and stridor was diagnosed as having Grade 3 Laryngomalacia and underwent Supraglottoplasty. Post-operatively, breathing and feeding improved, and child has started gaining weight.

6. A new born baby was diagnosed with Bilateral Bony Choanal Atresia. Child underwent surgery at 4 days of age - Trans nasal endoscopic Choanoplasty with drilling of bony obstruction was done. Stents were put in place for 2 weeks. Baby is doing well with large patent posterior nares after 1 month.

Last month we encountered a variety of Airway problems - from acute emergencies to chronic long standing problems. Sharing a few cases we managed successfully.

1. We perfomed OPD based Injection Laryngoplasty for a 70 year old lady suffering from Left Vocal Cord Paralysis. She had severe hoarseness of voice and was aspirating liquids.

2. We removed a metallic ring from the throat of a 6 month old baby.

3. Grade 3 Laryngomalacia in a 3 month old baby who had severe stridor and oxygen dependence was managed successfully.

4. An Internal Laryngocele was successfully operated by Endoscopic and Laser Trans-oral Laser Surgery.

5. A piece of Guava was removed from the Trachea of a 1 year old child

6. We removed pieces of Peanut lodged in the airway of a 2 year old baby.

We recently performed 2 challenging airway cases:
1. 4 year old baby - post Leptospirosis, hepatic encephalopathy, post Intubation Subglottic Stenosis. We did Trans-oral Endoscopic surgery and excised the stenosis successfully. Post-op 1 month endoscopy revealed normal lumen airway with no scarring. Baby is breathing normally now.
2. 2 month old baby presented with Aspiration Pneumonia secondary to Grade 3 Laryngomalacia. After Pneumonia was resolved, we performed Supraglottoplasty and the baby is doing well post-op.

Sharing an interesting and challenging case in which we managed post-intubation Acute Subglottic Stenosis in an 18 months-old baby. There was significant airway narrowing and the child had severe stridor. Subglottic Stenosis was diagnosed on the basis of bedside Flexible Awake Endoscopy. The child was a suspected case of Diphtheria and was managed expertly by the team of Paediatricians and Anaesthetist. The baby is doing well now.

We encountered some very interesting cases in the past few days.

1. 8 Year old girl was referred for Bronchoscopic evaluation of suspected Papilloma (suggested on CT scan). She had taken ATT for Pulmonary TB in the past year but her cough persisted. On awake OPD Bronchoscopy we found an Imli (Tamarind) Seed lodged in her Bronchus. The seed was subsequently removed in multiple pieces as it had been lodged for more than a year!

2. 15 month old baby was eating Badam (Almond) when he coughed and aspirated a small piece. He immediately had respiratory distress and was diagnosed clinically as having the Almond lodged in the Airway. It was removed as a single piece within hours of aspiration.

3. 11 month old baby aspirated a piece of Peanut 7 days back and had breathing difficulty. The foreign body was diagnosed on CT scan elsewhere and referred to us. It was removed piecemeal. This was the most difficult of the 3 surgeries as pieces of the peanut were lodged deep into the secondary bronchus and required a lot of patient instrumentation and manipulation to remove all the pieces.

These were 3 cases of the same pathology with 3 different presentations, and 3 different modes of diagnosis.

Foreign body aspiration is a very common cause of sudden onset respiratory distress and can present in a variety of ways. A high degree of clinical suspicion along with OPD based bronchoscopy evaluation can correctly diagnose the condition. Early detection allows for faster surgical removal and prevents development of a lot of comoridities to the children.

65 year old male patient accidentally ingested a sharp metal instrument while undergoing a dental procedure. He was brought to us within an hour of ingestion of the metal file. Xray confirmed its presence and the sharp pin like file was removed successfully from the throat of the patient. It was extremely thin and sharp and could have caused a lot of damage had it not been removed immediately. Sharing 4 different airway cases that we performed successfully over the last month.
1. 3 Year old baby was trying to blow a whistle when accidentally it slipped into his wind pipe and got lodged deep in his airway - at distal end of primary bronchus. It had 3 pieces and all were removed successfully.
2. 4 Year old baby got a piece of corn stuck in his wind pipe while trying to swallow it. He developed pneumonia and was diagnosed with having corn in his trachea by bedside flexible bronchoscopy. The corn kernel was then successfully removed.
3. 1 day old baby with complete white-out of left lung and requiring positive pressure oxygen support successfully underwent bedside flexible awake bronchoscopy and the airway was cleared. Air entry in left lung was restored.
4. 1 month old baby with decreased air entry in right upper lobe of lungs was treated with bedside flexible awake bronchoscopy.

We recently managed a variety of airway cases successfully.

1. A 2 year old kid with Safety pin stuck at floor of nose came with nasal bleeding. The pin was safely removed and child went home happy!

2. A 3 month old baby presented with Grade 3 Laryngomalacia and was successfully operated. We performed Supraglottoplasty and child was able to breathe and eat normally post operatively.

3. A 6 month old baby with severe stridor was found to have Secondary Laryngomalacia and Subglottic Hemangioma. Laser ablation of the Hemangioma along with Supraglottopplasty was performed and stridor resolved immediately.

4. A 15 day old baby was found to have Left sided large arytenoid mass with Left Vocal fold paralysis. The mass was removed successfully.

5. 2 children- a 3 year old boy and a 2 year old girl aspirated pieces of concrete and presented with respiratory distress. In one case the concrete piece was radio-opaque and clearly seen on Xray. Both successfully underwent Bronchoscopic removal of the offending foreign body and are doing well.

We managed 3 challenging airway cases successfully :

1. 8 month old 2.7kg baby - severe failure to thrive- presented with continuous stridor and was diagnosed as suffering from Grade 3 Laryngomalacia. Supraglottoplasty was done and a wide airway with no collapse was achieved.

2. 3 year old baby with history of 5 admissions for pneumonia with alternate lung collapse over last one month was diagnosed with flexible bronchoscopy - child had fragments of peanuts in both, right and left bronchus. All peanut pieces were successfully removed and child was discharged with complete resolution of Pneumonia.

3. 2 Year old baby with right side pneumonia and left subcutaneous emphysema was taken up for diagnostic bronchoscopy and found to have a piece of almond lodged in Right main Bronchus. It was removed successfully.
