RARE CASE OF SPIGELIAN HERNIA OPERATED LAPAROSCOPICALLY
42 yrs female came with large lower abdominal very big Hernia defect. CT scan revealed Large SPIGELIAN Hernia( Spigelian hernia is the type of ventral hernia that occurs through the Spigelian fascia, which is the part of the aponeurosis of the transverse abdominal muscle bounded by the linea semilunaris laterally and the lateral edge of the re**us abdominis muscle medially.)
Patient was operated Laparoscopically using TAPP ( trans abdominal preperitoneal ) approach. Large peritoneal flap lifted ,defect repaired with Barbed No.1 suture. Large 15X20 cm mesh placed and peritoneal defect closed with PDS 2-0.
Advantage of TAPP over IPOM
1. Mesh is not placed intraperitoneally hence the chances of adhesion to the intestine and complications are almost negligible.
2. Less painful and early recovery
3. Less cost
Disadvantage
1. No disadvantage to the patient, only more time consuming and tiring for the surgeon .
