We are happy to announce that we are starting Covid 19 vaccination in our hospital from 08.03.2021 on all working days between 10am to 4 pm .Please note the following instructions before you come for vaccination.

1. Please bring any one of the following 7 ID proofs to support your age for people born before 01.01.1962

Aadhar card
Electoral photo ID
Passport
Driving license
PAN card
NPR smart card
Pension document with photograph

2. Vaccination charges:250 rupees per person

3. Please wear mask and maintain social distance

4. 30 min observation is compulsory

5. Medical certificate in support of co morbidities for people born between 01.01.1962 and 01.01.1977 in prescribed format issued by a registered medical practitioner is mandatory

6. Health care workers and frontline workers, who have not taken/missed 1st or 2nd dose can also take vaccination. ID proof and employment certificate issued by the respective health care facility is mandatory
