Our one patient came with a complaint of urinary retention. Sonography revealed a 14 mm urethral calculus and a large, complete staghorn stone in the left kidney. He has been a frequent stone passer for a long time and has been seeking treatment for urethral stones for the last 2 months from four different states. After convincing him to have surgery, we went for an IVP, which showed poor excretion of dye and a faint nephrogram. We did cystolithotripsy and left PCNL in the same sitting. Complete C-arm clearance was achieved. Surgery time was approx. 3 hours with around 300 ml blood loss. Even in this era of ultra-mini PCNL and minimally invasive RIRS, we come across cases like these where we need dilatation of 26 and 28F.
