EBUS-TRANSBRONCHIAL LUNG NODULAR CRYOBIOPSY

This is our first case,,,,,

A 49-year-old female patient presented with impression of right lower lobe Pneumonia. CT THORAX- Well defined soft tissue density enhancing area noted involving superior and lateral basal segments of right lower lobe / Multiple enlarged enhancing mediastinal lymph node.

EBUS + TRANS NODULAR CRYOBIOPSY DONE: Main carina blunt. 4R, 7, 10R, and Small 10L are Visualised.
EBUS TBNA AND CORE BIOPSY TAKEN FROM 4R & 7.
EBUS-TRANSBRONCHIAL LUNG NODULAR CRYOBIOPSY TAKEN FROM 7 (SUBCARINAL LYMPH NODE)

SMOKING AND CONSUMPTION OF ALCOHOL IS INJURIOUS TO HEALTH🍻🚭

A 46-year-old male patient presented with complaints of chest pain and shortness of breath. It happened after an episode of binge drinking alcohol and this binge alcohol consumption led to accidental aspiration of his own upper incisor. The patient underwent rigid bronchoscopy and was found to have a foreign body (Tooth) impaction in right main bronchus, intending to intermediate bronchus. Removed in situ using forceps with Rigid bronchoscope 12 barrel.

Foreign body (Ground nut -Mixture) seen in basal segment of right lower lobe - removed using basket

On 21/03/2024

This 60-year-old female patient was on treatment from local hospital from were she developed increased breathing difficulty and stridor for which she was referred here for further evaluation and management. Diagnosed ADENOID CYSTIC CARCINOMA / STRIDOR / SVC OBSTRUCTION / RIGHT EXTENSIVE PARATRACHEAL LESION TO MAIN BRONCHUS AND CARINA / MULTINODULAR GOITRE WITH RETROSTERNAL EXTENSION / SYSTEMIC HYPERTENSION.
Y-stent insertion done under MAC and Fluoroscopy guidance through Flexible bronchoscopy. Main carina blunt. Right - Right main bronchus narrowed and erythematous. Can be negotiated with Paediatric bronchoscope with difficulty. Guidewire inserted to right main bronchus. Left main bronchus minimal narrowing noted. Guidewire inserted to left main bronchus. Y- Stent inserted through guidewire. Impression - Self-expanding Metallic Tracheobronchial Y Stent insertion done. Position checked under Fluoroscopy guidance. Post procedure patient was stable. After check bronchoscopy and Satisfactory dilatation achieved patient was discharged.

Self-expandable Metallic Stent (SEMS) Insertion.

On 26/02/2024

This 85-year-old male patient SCC Esophagus June 2023 - T2/3N2 ((R) I SCLN induced). Now presented with complaints of Hoarseness of voice increased dyspgagia, Aspiration, SOB increased. Bilateral vocal adducted. Extraluminal compression visualized from below subglottic area up to 3 cm above carina. Tracheoesophageal fistula visualized. Self-expandable metallic airway stents inserted successfully. Post procedure patient was stable.

A 37-year-old male who is a known case of metastatic Ewing Sarcoma right thigh presented with complaints of increased breathlessness and was diagnosed to have bilateral lung lesions with right moderate effusion. In view of advanced disease and respiratory failure he was planned for indwelling pleural catheter insertion as best treatment option corresponding to his clinical status. Pleural catheter insertion was done via Thoracoscopy guidance under IV sedation. Post procedure his symptoms improved ,repeat chest x ray showed expansion with catheter in situ and he was discharged with IPC. Indwelling pleural catheters (IPCs) are soft silicone tubes that are increasingly being used for patients with recurrent malignant pleural effusions. They are simple to place and can be done on an outpatient basis under local anaesthesia. IPCs uniformly relieve dyspnoea and improve quality of life of patients with malignant pleural effusions. Also allow people to manage the shortness of breath from recurrent malignant pleural effusions safely at home. It can also be used for Recurrent pleural effusion (RPE) which can be encountered in various benign conditions such as inflammatory, infectious, or other systemic diseases (e.g., congestive heart failure (CHF), hepatic Hydrothorax, post lung transplants, post coronary artery bypass graft (CABG) surgery, and chronic exudative pleurisy
