The cornea, often described as the 'window to the eye', is not only a structure of remarkable clarity but also a hub of sensory activity. Nestled within its layers are intricate networks of nerves that play a pivotal role in maintaining corneal health and function.
Usually these corneal nerves eludes visualisation on Slit lamp examination, but in this young female they were quite striking!
In otherwise Normal individuals, only the thickest stromal corneal nerves (axon bundles) can be seen on the slit lamp, typically in the peripheral one third of the cornea.
Enlarged or prominent corneal nerves seen in the central cornea may be an early hallmark of important systemic diseases like MEN2B and warrants further investigation.
These nerves should not be mistaken for corneal "ghost" blood vessels, which are characterised by having a lumen, making them appear as two parallel lines, with multiple fine and irregular branching.
