A challenging involving spondylolysis at L3 and high-grade spondylolytic listhesis at L4-L5 with severe foraminal stenosis.

Reduction of Listhesis here is a challenge, due to the high chances of screw pullout at L4. Also, placement of a cage for foraminal height restoration can be counterproductive due to endplate damage in a collapsed disc space causing cage subsidence later.

In this case, to solve the issue, we placed 7 mm bicortical pedicle screws at L3 and L5 and fenestrated screws in L4 for cement augmentation. The augmented vertebra was then reduced resulting in restoration of foraminal height. posterolateral fusion was then performed and the patient is recovering fast

I am very happy and proud that a mammoth project of 51 chapters from leading national and international and has now been published in our “Pediatric Scloiosis” textbook under the prestigious Springer group.

It took 2 years of hard work. I thank my co-editors Balaji Zacharia and Nikhil KV for the wonderful academic journey. I thank Kauvery Hospital for the support.
