Vertebro basilar insufficiency [ VBI ] , reduced blood supply to behind part of brain , smaller brain [ cerebellum] and brain stem due to narrowing [ stenosis ] of blood vessels supplying that particular area of brain .
With above clinical situation patient could be suffering from , loss of balance while walking , dizziness or vertigo , delirium [ confusion ], difficulty in swallowing [ dysphagia ],lack of coordination of extremities , double vision or recurrent fainting attacks.
50 year old genteman , manual labourer , smoker presented to me with recurrent attacks of fall and unable to walk due to imbalance .
MRI brain (performed in another hospital) showed normal brain parenchyma but suspiciously invisible / unclear segments of arteries supplying brain stem . Clinical examination revealed tell tale signs of vertebro basilar insufficiency
digital subraction angiography was done which showed severely stenosed [ narrowing ] vertebral artery [ The artery which supplies the critical areas of behind part of brain including brain stem].
Advances in Neuro interventional procedures allow us to treat these serious situations safely by angioplasty in brain arteries . similar to coronary angioplasty
