IMPORTANT: Not all heel pains are Plantar Fascitis...❌

Heel pain is an extremely common complaint with several common causes.

1)Plantar Fascitis (is the most common cause) - Irritation and swelling of the thick tissue on the bottom of the foot. It is more severe in the morning. Symptoms intensify with prolonged standing or exercises. Plantar Fascitis is usually associated with a tight heel cord.

2)Baxters nerve compression ( a rare cause) - It is the 1st branch of lateral plantar nerve. The Baxters nerve contributes to 20% of heel pain cases. The nerve provides motor innervation to abductor digiti minimi muscle. The involvement of Baxters nerve may affect running athletes causing pain on the medial plantar aspect of foot. Entrapment of Baxters nerve is usually overlooked or misdiagnosed.

3) Fat pad atrophy - The fat pad cushions the calcaneus and with age this fat pad atrophies( starts thinning) thus causing significant pain while walking in elderly population. The pain is deep, central, non radiating plantar heel pain that is worsened when the person is barefoot and desolves when the person walks on his/her toes with tenderness on the central aspect of the heel pad.

4) Stress fractures of calcaneus - This can occur due to overuse injuries. The person will complain of heel pain which increases with walking/running and does not subside throughout the day. The pain is more with medial to lateral compression of calcaneal tuberosity.

5) Tarsal tunnel syndrome - Paresthesia/ numbness in the plantar foot. Symptoms are worse with activity and the Paresthesia may wake the person up at night.

6) Lumbosacral spine radiculopathy - Pain in the lateral side of the foot can result from L5,S1 herniated disk. The S1 nerve root causes decreases sensation and pain on the lateral aspect of the foot.

7) Achilles tendinitis - It's a common overuse injury of the Achilles tendon- the band that connects the calf muscles at the back of the lower leg to the heel bone. Tenderness/stiffness especially in the morning that improves with mild activity. Seen more common in runners and middle aged people.
