✨Advancing the Fight Against Gastric Cancer with Peritoneal Metastases✨
🌟Today, I am reflecting on a profoundly complex and hopeful case that exemplifies the progress we are making in surgical oncology. We recently had the privilege of treating a 39-year-old female patient with aggressive gastric cancer that had spread to her peritoneal cavity—a diagnosis historically considered untreatable with very poor prognosis.
After more than six months of multi-line chemotherapy (including FLOT) and three staging laparoscopies, her disease remained stable. This careful selection and preparation allowed us to proceed with a highly specialized and radical procedure: Cytoreductive Surgery (CRS) and Hyperthermic Intraperitoneal Chemotherapy (HIPEC).
The extensive surgery included a total gastrectomy, hysterectomy with bilateral salpingo-oophorectomy (TAH BSO), and peritonectomy to remove all visible disease. I am thrilled to share that her postoperative recovery has been excellent, and the final pathology showed a T3 N0 result.
This case is a testament to the importance of a multi-disciplinary team, meticulous patient selection (using indices like the PCI), and never giving up on potential candidates. While peritoneal disease from gastric cancer remains a formidable challenge, selected cases like this one can now have a chance at long-term survival and hope.
