A torn meniscus is one of the most common knee injuries. Any activity that causes you to forcefully twist or rotate your knee, especially when putting your full weight on it, can lead to a torn meniscus.

Each of your knees has two C-shaped pieces of cartilage that act like a cushion between your shinbone and your thighbone. A torn meniscus causes pain, swelling and stiffness.

The meniscus is a C-shaped piece of tough, rubbery cartilage that acts as a shock absorber between the shinbone and the thighbone. It can be torn if you suddenly twist your knee while bearing weight on it.

A calf strain is the overstretching or tearing of either of the 2 calf muscles. Other possible causes of calf pain include: Achilles tendon injuries such as Achilles tendinopathy or a sudden rupture of your Achilles tendon.

How to reduce it?

To alleviate calf pain, perform gentle exercises like the wall calf stretch (keeping the back leg straight and heel down) to target the gastrocnemius muscle, and the bent-knee calf stretch (soleus stretch) to focus on the soleus muscle. Strengthen the calf by performing slow, controlled heel raises, starting with both feet on the floor and progressing to single-leg raises on a step, and consider using a foam roller to release muscle tension. Remember to listen to your body, stop if you feel pain, and consult a healthcare professional for persistent or severe calf pain.

Stretches for the Gastrocnemius

🔷Wall Calf Stretch:

Stand a few feet from a wall, place the ball of your foot against it, and lean forward, keeping your back leg straight and heel on the floor to feel a stretch in the upper calf.

🔷Downward-Facing Dog:

A yoga pose that stretches the entire back of the leg, including the calf.

🔷Lunge Stretch:

Step into a lunge position with one foot back. Keeping the back heel on the floor and knee straight, lean into the wall.

Stretches for the Soleus

🔷Bent-Knee Calf Stretch:

With your foot in a similar position to the wall calf stretch, bend your back knee while keeping your heel on the floor. This will create a stretch lower in the calf.

Strengthening Exercises

🔷Heel Raises (Double-Leg):

Stand on flat ground, hold a wall for balance, and slowly lift both heels off the ground, coming onto the balls of your feet.

🔷Heel Raises (Single-Leg):

Once comfortable, progress to performing heel raises on one leg at a time to increase the load.

🔷Single-Leg Heel Raises on a Step:

Stand on a step and lower your heel below the level of the step for a greater range of motion and stretch.

Other Techniques

🔷Foam Rolling:

Place a foam roller under your calf and use your upper body to roll back and forth, or cross one leg over the other for increased intensity.

Electrode Placement: Place the electrodes on the skin surface over the target muscles using adhesive patches. The placement will vary depending on the specific muscles being targeted for stimulation. In the case of foot drop, the electrodes are typically positioned to stimulate the muscles involved in dorsiflexion of the foot, which is the movement of pulling the foot upward.

a. Dorsiflexors: The electrodes are usually placed over the anterior tibialis muscle, which is the primary muscle responsible for dorsiflexion of the foot. Position one electrode on the front of the leg, slightly below the knee, and the other electrode on the front of the ankle, near the area of the anterior tibialis muscle belly.

The straight leg raise is a test that can be performed during a physical examination, with the leg being lifted actively by the patient or passively by the clinician. If the straight leg raise is done actively by the patient it is a test of functional leg strength, particularly the re**us femoris element of the quadriceps (checking both hip flexion and knee extension strength simultaneously).

If carried out passively (also called Lasègue's sign, Lasègue test or Lazarević's sign), it is used to determine whether a patient with low back pain has an underlying nerve root sensitivity, often located at L5 (fifth lumbar spinal nerve).

If the patient experiences sciatic pain, more specifically pain radiating down the leg (radiculopathy), when the straight leg is at an angle of between 30 and 70 degrees, then the test is positive and a herniated disk is a possible cause of the pain.

Remember you can get this angle using the treatment plane as 0 degree or the starting point.

Motor-related symptoms

Slowed movements (bradykinesia). A Parkinson’s disease diagnosis requires that you have this symptom. People who have this describe it as muscle weakness, but it happens because of muscle control problems, and there’s no actual loss of strength.

Tremor while muscles are at rest. This is a rhythmic shaking of muscles even when you’re not using them and happens in about 80% of Parkinson’s disease cases. Resting tremors are different from essential tremors, which don’t usually happen when muscles are at rest.

Rigidity or stiffness. Lead-pipe rigidity and cogwheel stiffness are common symptoms of Parkinson’s disease. Lead-pipe rigidity is a constant, unchanging stiffness when moving a body part. Cogwheel stiffness happens when you combine tremor and lead-pipe rigidity. It gets its name because of the jerky, stop-and-go appearance of the movements (think of it as the second hand on a mechanical clock).

Unstable posture or walking gait. The slowed movements and stiffness of Parkinson’s disease cause a hunched over or stooped stance. This usually appears as the disease gets worse. It’s visible when a person walks because they’ll use shorter, shuffling strides and move their arms less. Turning while walking may take several steps.

Additional motor symptoms can include:
Blinking less often than usual. This is also a symptom of reduced control of facial muscles.

Cramped or small handwriting. Known as micrographia, this happens because of muscle control problems.

Drooling. Another symptom that happens because of loss of facial muscle control.

Mask-like facial expression. Known as hypomimia, this means facial expressions change very little or not at all.

Trouble swallowing (dysphagia). This happens with reduced throat muscle control. It increases the risk of problems like pneumonia or choking.
Unusually soft speaking voice (hypophonia). This happens because of reduced muscle control in the throat and chest.

There are three main types of Spina Bifida.

1. Spina Bifida Occulta:
This is the mildest and most common form of spina bifida. In this condition, the spinal column doesn't fully close, but the spinal cord and the nerves usually remain undamaged. Often, there are no visible signs on the skin surface, and many people with spina bifida occulta may not even be aware they have it unless it's discovered during an imaging test for an unrelated issue.

2. Meningocele:
In this type, a sac containing the protective covering of the spinal cord (meninges) protrudes through the opening in the spine, forming a visible lump or sac on the back. The spinal cord and nerves are typically not involved, so individuals with meningocele may have minimal or no neurological symptoms. Surgical intervention is often required to repair the opening and remove the sac.

3. Myelomeningocele:
This is the most severe form of spina bifida. In myelomeningocele, the spinal canal remains open along several vertebrae, allowing both the spinal cord and the protective covering (meninges) to protrude through the gap, forming a sac or cyst on the back. The spinal cord and nerves are exposed and often damaged, leading to varying degrees of neurological impairment, such as paralysis, sensory deficits, and bowel and bladder dysfunction. Surgery is typically performed shortly after birth to close the opening and protect the spinal cord from further damage, but lifelong management is often necessary to address the associated complications.

When a patient has flatback syndrome they have lost the normal amount of lordosis, or inward curve of the spine so the body adapts by tilting the pelvis, flexing the knees and hips so the patient can maintain an uprigh posture.

Often patients present for therapy because of other associated symptoms including back or leg pain, difficulty with doing certain daily activities, and sensation of falling forward.

Patients may notice worsening posture over time and even sometimes have started to use a cane or walking stick to help with ambulation. Symptoms often worsen as the day progresses because the body is overcompensating leading to excess muscular fatigue. In some cases, patients with underlying flatback syndrome also have spinal stenosis which is a narrowing around the spinal canal which can cause other nerve symptoms.

The cause of flat back syndrome can either be of these reasons;
1. Congenital (present at birth)
2. Ankylosing spondylitis (a type of inflammatory arthritis of the spine)
3 Compression fractures of the vertebrae, such as due to osteoporosis
4. Degenerative disc disease

5. Flat back syndrome can occur after spine surgery, such as laminectomy. In the past, surgically implanted devices used to correct scoliosis often flattened the lumbar spine and resulted in flat back syndrome, especially with age. Updated surgical techniques have reduced this complication.

Managing the flat back syndrome mainly becomes more successful with physiotherapy. Exercises designed to help reverse the changes back to normal can be recommended by your therapist.

However, even before your therapist handles you, an X-ray of your lumbar spine will be requested to establish the presence of the condition. This can be confirmed when there is loss of the lordosis curve which naturally exist at every individual’s lower back.

When this is ruled out, then many of our protocols will be that which will help restore the normal lordotic curve, which in turns will correct all other changes to the other nearby joints and we also focus on symptom related management to set you free. These protocols will be but not limited to exercise therapy.
