NEVER IGNORE BLOOD IN MOTIONS !

It can be piles most of the times , but sometimes it can be a serious condition also !

56 yr old lady , known diabetic presented with bleeding per re**um ( blood in motion) for 1 yr duration with loss of weight and appetite for 2 months . She however sought medical attention recently only . She was evaluated with colonoscopy and CECT abdomen which revealed a long growth in re**um and the biopsy was reported as cancer of large intestine ( re**um) . MRI pelvis also confirmed the findings and the cancer was deemed operable without much spread . As it was low re**al cancer , planned for abdominoperineal resection operation. She was adequately counselled regarding a permanent colostomy ( motion will get collected in a bag rather than passing from the a**l opening ) . It is a major surgery involving 2 phases , the abdominal & perineal phase . We did the abdominal part by laparoscopy resulting in only 1 stitch line . Patient recovered well and walked independently on the third postoperative day . She had a smooth recovery and was completely fit for discharge by fifth post operative day .

Though it is a major surgery , recovery was smooth because of the minimally invasive approach . Few small scars in the abdomen with only incision being required for stoma helped in reducing pain and postoperative wound infection with better cosmesis. The final biopsy was early cancer and she doesn't require any further treatment like chemotherapy or radiation .

Non alcoholic steato hepatitis is the technical term for the commonly seen ' Fatty liver' with liver damage .

With frequent use of Ultrasound and being easily available , many patients are diagnosed with fatty liver . Even young , healthy appearing people are diagnosed to have it . But not all require to be concerned . Only few people with fatty liver develop liver damage ( NASH ) , however if left untreated in the long term can cause severe liver damage with liver failure over a period of 10-20 years .

It is usually asymptomatic . Some people can complain of weakness or tiredness . In case abdominal symptoms are there in a patient with fatty liver , we need to look for other probable causes of pain as it seldom causes pain or other disturbances.

Diabetes , high cholesterol , hypothyroidism and obesity are the main risk factors . Alcohol consumption will aggravate the condition with further damage to liver .

The mainstay of treatment of fatty liver is Dietary modifications and lifestyle changes ( physical activity etc) . Only a few patients require specific medications for fatty liver . However , NASH should always be treated early to avoid possible liver failure in the future .
