Oral cancer and an oral and maxillofacial surgeon

Oral and maxillofacial surgeons role in treating oral cancer. : After dedicated surgical training in oral surgical oncology he/she can be the primary surgeon or an OMF surgeon can be the first attending surgeon along with a primary consultant for treating an oral cancer patient .

Apart from regular surgical treatment and follow up, role of omfs in cases where micro vascular free flap are used for reconstruction is very important. First 2 days post op , flap monitoring is very vital and identifying flap failures quickly can help to salvage free flaps and saving extreme morbidity for patient. Specially cases of fibula free flap, when large defects are closed ! Identifying flap failures or change in vitality of flap is very critical . And decision making to take the patient again to the theatre is essential.

Here is a case of fibula free flap which was used to reconstruct a huge defect of Right composite resection . At pod -1 there was a mild change in color of flap with bluish discolouration at flap suture ends and margins. Further which in few hours flap bleed turned into darker red and blackish corresponding to a venous stasis or block in the venous Anastomosis.
Neck was soft still no pressure laid over the anastomoses site .. other vital parameters where altered and altering position of neck of patient did not give sufficient result in clinical appearance of flap . Hence it was decided to re explore the anastamosis , the attending plastic surgeon was informed and prior to which patient was shifted to theatre and neck exposed , to see
1. huge clot present at the venous anastomoses site ( venae comitants of peroneal vessel). Ijv was used for flap anastomoses! video shows huge clot totally obstructing the venous drainage and when released the release of huge clot can be noticed

2. Anastomoses was done again by the reconstructive surgeon and flap was saved

Monitoring free flaps and decision making for flap re exploration is very important in cases of flap reconstruction after oral cancer surgeries
