Anurag Hospital is Centre for treatment of surgical disorders with ambulatory surgical care and also for Obesity and Diabetes Surgery and Support. It was started by Dr John AC Thanakumar, an expert and pioneering advanced laparoscopic and bariatric surgeon who has been awarded by the former President of India Dr K R Narayanan during his tenure. Dr John AC Thanakumar is the President of Indian Association of Gastrointestinal Endosurgeons of India (IAGES) , body that comprises of nearly 4000 laparoscopic surgeons all over India.His earlier basic surgical training and later masters were in Christian Medical College, Vellore. Since then he has been trained in Germany, France, USA. He has contributed to laparoscopic surgery and obesity surgery by way of scientific articles, books and as operating faculty in India and abroad. Dr Pravin John in 3cm paraumbilical hernia with bowel and fat contents demonstrates the suturing part in eTEP-RS surgery for paraumbilical hernia. Only the suturing techniques are described in this video. https://www.youtube.com/watch?v=sRjZABkaZ8M

In the operation of eTEP RS for paraumbilical hernia repair, suturing is needed in the following areas:
1) Peritoneal closure after reduction of hernia contents. The posterior re**us sheath (PRS) is not always to be included in this suture.
2)Peritoneal defects inadvertently created have to be closed to prevent bowel adhesions to the exposed polypropylene mesh
3)Closure of the anterior fascia with optional inclusion of the hernia sac on the roof-top with correction of recti divarication can be taxing. Hence this needs special mention.

Tips for suturing with prolene in eTEP RS
1. Keep the length of the suture to 15-18 cm
2. A hangman’s knot made extracorporeally makes a convenient loop for the needle to pass through to tighten the first knot which must be succeeded by several subsequent knots OR
3. While initiating the first knot at the outset, one needs to consider a granny knot with three loops rather than the conventional 2 loops as three loops will keep the knot in situ snug while the subsequent knots are in place OR
4. A sliding knot can also be substituted instead of step three as it slides easily with prolene monofilament suture.
5. Steps 2,3 and 4 are especially useful while suturing on the roof to oppose the anterior fascial defect or while correcting the recti divarication. This can be aided a lot by bringing down deliberately the intra-abdominal pressure (IAP) to much lower levels to aid the suture to be in place.
6. While doing a continuous suture, one must remember to use the tissue to be pushed back so that the tension on the suture is maintained
7. Barbed sutures are available as an easy option, but with good suturing techniques described above, the results will be good
