Proximal Caries – Overview

Definition:
Proximal caries are dental caries (tooth decay) that occur on the smooth surfaces between two adjacent teeth, usually on the mesial (toward the midline) or distal (away from the midline) surfaces. These areas are difficult to clean, making them prone to decay.

🦷 Causes:

1. Plaque accumulation in between teeth where brushing is less effective.

2. High sugar intake – bacteria convert sugars into acids that demineralize enamel.

3. Poor oral hygiene – not flossing or using interdental cleaners.

4. Tight tooth contacts – make cleaning the area difficult.

🔍 Clinical Features:

May not be visible in early stages.

Can cause discoloration or shadowing between teeth.

Food lodgment or discomfort when chewing.

Sensitivity to sweet, cold, or hot foods as it progresses.

Advanced lesions may cause pain or lead to pulp involvement.

🧪 Diagnosis:

Bitewing radiographs – best for detecting early proximal caries.

Clinical examination using mirror and explorer.

Fiber optic transillumination (FOTI) – helps detect lesions by light transmission.

💊 Management:

Early stage (incipient):

Remineralization with fluoride varnish, fluoride toothpaste, or remineralizing agents.

Improve oral hygiene and diet control.

Advanced stage (cavitated):

Restoration with composite resin or glass ionomer cement.

If extensive: may require crown or root canal treatment.
