Sometimes patients surprise you with the good outcome when you don’t have much expectation of recovery. Such was the story of this 25 years old gentleman who met with a bike accident and sustained a major crush injury of the right foot with all the toes coming out as socks with shaft femur and distal femur hoffa fracture on same side. He also had a traumatic brain injury with altered sensorium and was managed at neuro ICU by our neurosurgery team. We could operate on him the next day as initially his general condition was not stable. We did thorough debridement of the foot and we could not save the toes, we we tried our best to save the foot. We also did femur retrograde nailing and hoffa fracture fixation. Then his routine dressing was done by my assistant Subhodeep, who did an excellent job and he also applied VAC (state of the art vacuum assisted closure device) for better healing. Then we did an SSG (split skin grafting, a plastic surgical procedure to cover raw area by skin, harvested from another site). He recovered very well. At 3 months he shocked me when he came to my clinic walking. He is the story of absolute resilience.I heartily thank all the people who were involved during his treatment including the ICU staffs, anaesthesiologists, OT team members.
