Headache is a very common symptom which brings a patient to a Doctor. It can be due to multiple reasons. The most common headaches are migraine and tension headaches. Although it can also be because of a brain tumour, hemorrhage, infections diseases like meningitis, encephalitis, tuberculosis, hydrocephalus etc. Sometimes headache may be a symptom of raised blood pressure. If headache is very severe, sudden, acute onset, associated with vomiting, paralysis, deviation of mouth, seizure, deteriorating vision, one should seek the advice of a neurologist and if needed or felt necessary then brain imaging studies like CT Scan or MRI Scan should be done. A chronic headache which becomes progressively more frequent and severe also needs a brain scan. In any patient of trauma or road traffic accident associated with loss of consciousness, multiple vomitings, paralysis or weakness in a limb, ear or nose bleeding, seizure, altered sensorium or in a case of high velocity road traffic accidents a brain imaging study should be done.

In treatment of migraine and other chronic headaches patients should also make some life style changes like regular exercise, regular balanced diet, drinking plenty of water to maintain hydration, avoid mental and physical stress, avoid any alcohol or to***co or any drug abuse, avoid excessive use of mobile phones and social media. Also patients should inculcate a habit of daily meditation and maintain mental calm and peace.Sometimes a sharp shooting pain in facial region and head also occurs in Trigeminal Neurslgia. Stay healthy and stay safe.

कुछ पल याद आ रहे हैं, कुछ यादें, याद आ रही हैं। वे दिन जब दिन रात एक करके अकेले दम पर दरभंगा जैसे छोटे से एक शहर में न्यूरोसर्जरी की शुरूआत की थी। पर जिंदगी कहाँ रुकती है। एक नदी की तरह बहती चली जाती है। भारत माँ की सेवा में समर्पित होकर जीवन का हर पल सफल है।

Sabhi ko Namaste. I Wish to Spread Awareness about Importance of Mental Health in the Society.
In today's world where everyday life has become a struggle for survival, there are too few people who can act as confidante or reliable relationships to share thoughts and emotions. This has led to a massive increase in problems related to mental health leading to sadness, mood disorders and psychosomatic complaints. Over engagement in social media and screen time are also major contributors to poor mental health. Just imagine the plight of our society. The earning member of the family is worried, how he or she will earn the livelihood! The house maker (usually a lady in our Indian Society) is worried about the expenses of running the household anf future of children. Children inschool are worried about college and children in college are worried about career and relationships. And worst of all we are not ready to acknowledge the existence of this problem. It happens everyday in my OPD patients, some of whom, after taking history from them I feel that they are not suffering from any physical disease but are suffering from a psychosomatic problem and when I ask what mental stress factors did you have which triggered these symptoms? Majority of them will answer - Nothing!!!! I really have hard time explaining to my patients that physical disease symptoms can also arise from mental stress factors without any disease process in the body. We should spread awareness about the importance of mental health in our Society.
We should include meditation, yoga and pranayama in our daily lives to mitigate these issues effectively. We should decrease the use of social media and screentime to minimum possible. And although we cannot avoid mental stress altogether in our lives we must strive to minimise it as far as we can.

नमस्कार, मैं सभी देशवासियों का ध्यान सड़क सुरक्षा की तरफ़ लाना चाहता हूँ। एक न्यूरोसर्जन (ब्रेन एवं स्पाइनल सर्जन) होने के नाते मैं प्रत्येक दिन मरीज़ों को हताहत या घायल होते हुए देखता हूँ।
हममें से शायद ही कोई ऐसा होगा (स्वयं मैं भी) जो किसी सड़क हादसे में बाल बाल ना बचा हो।

प्रत्येक दिन पूरे देश में सड़क दुर्घटनाओं में सैकड़ों लोग मारे जाते हैं। सड़कें किसी भी महामारी , बीमारी, युद्ध से ज़्यादा बड़ी मृत्यु स्थल बन चुकी हैं लेकिन हम भारत के लोगों की सामूहिक चेतना नहीं जागती !!!

दुर्घटना अमीर, ग़रीब, पद कुछ नहीं देखती। शायद ही ऐसा कोई परिवार होगा जिसने परिवार में किसी के ना खोया हो। परंतु आश्चर्य इस बात का होता है कि भारत देश में सड़क पर चलने में सावधानी बरतने की कोई जागरूकता नहीं है बल्कि यह दिन प्रतिदिन और कम होती जा रही है। चाहे पैदल हो, दुपहिया वाहन, टेम्पो, ऑटो, ट्रक बस, कार , कुछ भी हो हम सड़क पर हैं इसका होश ही नहीं है। बमुश्किल कुछ एकाध प्रतिशत लोग समझ रखते हैं कि सड़क; सड़क होती है घर का आँगन नहीं!

मोबाइल पर बात करते हुए रोड पर चलना, कितना भी हॉर्न बजाओ रास्ते से ना हटना, बाएँ साइड इंडिकेटर देकर बाएँ मुड़ रही कार को बाएँ से चौराहे पर ओवरटेक करना, बाएं से तेज रफ़्तार से ओवरटेक करके दायें रास्ते में बिना इंडिकेटर दिए घुस जाना, आगे चलते हुए वाहन से दूरी बनाकर नहीं चलना, सड़क छेंककर वाहन खड़े करना, गलत साइड से चलना आदि आदि।
ये हमारे राष्ट्रीय मौलिक संवैधानिक अधिकार जैसा बन गया हैं और ये हमारा पिछले बीस वर्षों का बौद्धिक विकास है।
केवल सड़क, मॉल, बिल्डिंग बनाने से देश आगे नहीं बढ़ता। हमें यह समझना होगा कि देश आगे बढ़ता है बौद्धिक विकास, शिक्षा के स्तर, शिक्षा के सम्मान और राष्ट्रीय चेतना से।

Mental Health is a very important and serious issue in today’s World. मानसिक स्वास्थ्य आज की दुनिया में मानसिक स्वास्थ्य एक गम्भीर वैश्विक समस्या बन गया है। शारीरिक स्वास्थ्य पर तो हम ध्यान दे भी देते हैं लेकिन मानसिक स्वास्थ्य बहुसंख्यक लोगों द्वारा उपेक्षित है। मानसिक अवसाद (Depression) एक महामारी का स्वरूप ले चुकी है। बतौर चिकित्सक होने के नाते मैं समाज में छोटे छोटे-छोटे बच्चों को भी मानसिक अवसाद का शिकार होते हुए देख रहा हूँ। हमें मानसिक स्वास्थ्य के प्रति जागरूक होने की ज़रूरत है। योग ध्यान और प्राणायाम को हमारे जीवन शैली में शामिल करना बेहद ज़रूरी है। मानव मन के तीन स्तर होते हैं (चेतन मन - conscious mind, अवचेतन मन - subconscious mind, अचेतन मन - unconscious mind). यदि हम चेसन मन में हमेशा चिंताग्रस्त रहेंगे तो यह हमारे अवचेतन और अचेतन मन में प्रवेश कर जाएगा और हम कब मानसिक अवसाद का शिकार हो जाएंगे हमें पता भी नहीं चलेगा। और फिर जन्म लेंगी बीमारियां - डायबिटीज़, हाई ब्लड प्रेशर, दिल की बीमारी, ब्रेन स्ट्रोक या लकवा आदि आदि। वर्तमान समाजिक जीवन में तनाव को पूरी तरह से ख़त्म तो हम नहीं कर सकते हैं परन्तु इसे कम करने का प्रयास हमें ज़रूर करना चाहिए। और योग, प्राणायाम, ध्यान (Meditation) इसके लिये बहुत ही महत्वपूर्ण हैं।

कमर दर्द एक बहुत आम समस्या है जो कभी ना कभी हम सभी को परेशान करती है। सामान्य तौर पर यह हमारे उठने बैठने चलने या भारी सामान उठाते समय गलत posture या तरीक़े के कारण पीठ की मांसपेशियों में खिंचाव के कारण होता है जो कुछ दिनों में सही इलाज लेने से ठीक हो जाता है । परंतु यदि यह दर्द बहुत ज़्यादा हो, पैरों में भी जा रहा हो, पैरों में झुनझुनाहट या कमजोरी के साथ हो तो न्यूरोसर्जन से ज़रूर सलाह लें विशेष रूप से यदि पेशाब पर नियंत्रण रखने में कठिनाई महसूस हो रही हो तो भी जल्द से जल्द स्पेशलिस्ट डॉक्टर से संपर्क करें। कभी कभी यह पैरों मे जाने वाला दर्द खड़े होने या चलने पर बढ़ता है । ऐसे में यह nerves पर दबाव के कारण हो सकता है । कभी-कभी डायबिटीज़ के मरीज़ों में भी यह समस्या देखने को मिलती है । इसके अलावा कहीं चोट लगने या गिरने पर यदि कमर में दर्द हो तो भी डॉक्टर से सलाह ज़रूर लें ।
कभी ऐसा स्लिप डिस्क, स्पॉन्डाइलोसिस, स्पाइनल ट्यूमर या टी बी के इंफ़ेक्शन के कारण भी हो सकता है । ऐसे में चिकित्सक MRI Scan भी करवा सकते हैं । सही समय पर सही इलाज से भविष्य में होने वाली परेशानी एवं complications से बचा जा सकता है । विशेष सलाह के लिए मेरे पटना क्लीनिक पर भी आप संपर्क कर सकते हैं (7303140884)

This is a CT Scan photo of a patient (identity not revealed) with brain hemorrhage (circled with red line). The patient is a 57 year old gentleman with undiagnosed hypertension. It's sad reality that most of the patients of brain stroke or heart attack are suffering from either undiagnosed hypertension, diabetes or hypercholesterolemia or even if diagnosed majority are taking irregular medications or are not in regular follow up with physician. These patients fallaciously consider themselves in good health and tragically land up in emergency with life threatening complication of Brain Stroke, Hemorrhage or Heart attack. It becomes a time taking and costly exercise often with poor prognosis and recovery. This pic shows a Brain hemorrhage which may even require Brain surgery. Please promote preventive healthcare and regular health check ups after the age of 35 years or earlier if there is someone in family suffering with this disease. If diagnosed as hypertensive or diabetic then please remain under regular treatment and follow up of a physician to prevent this kind f a disaster.

Trust & Respect Form the Backbone of Doctor Patient Relationship.
A Doctor is a healer. He / she sacrifices the best years of youth of his life studying in a Medical College to learn the art and science of healing. I personally spent 14 years of my life from 6th of August 1997 (admission into MBBS) to 31st of July 2011 (Finishing my MCh Neurosurgery), passing every examination in my first attempt. Despite being the only child of my parents, I spent all these years of my life in solitude studying medicine. Definitely and decidedly no remuneration can pay me back for the years of my life I sacrificed for this nobel profession. Sadly the times have changed and the nature of Doctor Patient relationship has also changed drastically. Everything today is weighed in terms of monetary exchange and it is believed that money can buy everything. Unfortunately this is far from reality. As a Doctor I come across numerous terminally ill patients from all socioeconomic backgrounds and sadly the entire wealth present upon Earth and entire knowledge of science on this planet cannot buy a single breath for the dying person. The success of Doctor as a healer depends upon a healthy Doctor - Patient relationship based upon mutual trust and respect which no amount of money can buy and no number of 5 star Google reviews can generate. People give reviews to temples as well as prisons. Although we have to live in the same society of present times but I as an old fashioned Doctor feel uncomfortable with the way a Doctor - Patient relationship is seen both by Doctors, Hospitals and Patients alike. Clealy there is something amiss somewhere.
