ULTRASOUND is very important in early diagnosis of PCOS.

Polycystic ovarian syndrome (PCOS), recently referred also as hyperandrogenic anovulation, is a chronic anovulation syndrome associated with androgen excess.

The classic triad of PCOS is:
oligomenorrhea and/or

anovulation
hirsutism
obesity

In addition to this, patients may have infertility, acne, alopecia or biochemically show increased androgen levels.
Biochemical hyperandrogenism is based on the measurement of free testosterone, free androgen index, or calculated bioavailable testosterone, androstenedione and dehydroepiandrosterone sulphate.

Anti-Müllerian hormone (AMH) levels are generally increased, and there is emerging evidence for the utility of AMH in the diagnosis of PCOS. At the time of writing, there is insufficient assay standardization and validation, and AMH should not be used in the diagnosis of PCOS at this stage.

ULTRASOUND IN ECTOPIC PREGNANCY

Ectopic pregnancy is one of the obstetric emergencies with long-term morbidity and mortality. Hence, high degree of suspicion, early diagnosis, and treatment improves the future reproductive potential. Ultrasonography helps in early diagnosis. Hence, all early pregnancies should undergo Ultrasonography for viability and site of pregnancy. It can be considered as the gold standard in the diagnosis of ectopic pregnancy. It serves as single most, non-invasive, diagnostic test. It can be even used as single alone test.

The risk of VTE in COVID-19 is high:

Due to high incidence of DVT

Pulmonary embolism (PE) and clotting in renal replacement circuits in COVID-19 ICU patients are early and late complications.

COVID-19 is associated with immunothrombotic dysregulation. This manifests with high D-dimer, high C-reactive protein levels, anti-phospholipid antibodies and sepsis-induced coagulopathy and is likely to increase mortality.

Screening for coagulopathy can risk stratify patients and may determine the need for anticoagulation. However, higher D-dimer cutoffs may be needed to improve its specificity for DVT in COVID-19.

Whether DVT detection at hospital admission suggests more severe COVID-19 remains unknown.

Despite standard thromboprophylaxis DVT is common in COVID-19

Evidence
Ultrasound is the mainstay of DVT diagnosis. Screening is advised, when feasible, in the general management of COVID-19 patients. Although routine screening is not widely recommended, twice weekly ultrasound surveillance can detect DVT, avert PE and reduce mortality in ICU patients.

Lower extremity ultrasound is recommended in COVID-19 patients with unexplained RV dysfunction, unexplained/refractory hypoxemia, or in patients with suspected PE who are too unstable for intra-hospital transport.
