Piles or Hemorrhoids(Arsha)

Haemorrhoids or piles are dilated or swollen veins (low pressure blood vessels that return blood to the heart) under the re**um around the a**l opening.

They occur frequently in the adult general population. Notably, a considerable number of people with hemorrhoids do not complain about symptoms. Many people have hemorrhoids at some point in their lives. Over 50% of the population, in fact, will experience this condition. The prevalence of hemorrhoids is equal between men and women but men are more likely to seek treatment.

Causes of Hemorrhoids or Piles(Arsha)

The exact cause of hemorrhoids is unclear, but they’re associated with increased pressure in the blood vessels in and around your a**s. This pressure can cause the blood vessels in the re**um and a**s to become swollen and inflamed.
Factors predisposing to the development of hemorrhoids include:
o irregular bowel habits (constipation or diarrhoea)
o Sitting for long periods of time can start the hemorrhoidic process
o increased intra-abdominal pressure (prolonged straining)
o a low-fiber diet
o Obesity can be a factor by increasing re**al vein pressure. Poor muscle tone or poor posture can result in too much pressure on the re**al veins
o the absence of valves within the haemorrhoidal veins
o aging: as we grow older the body’s supporting tissues get weaker
o being overweight or obese
o Increased straining during bowel movements caused by constipation or diarrhea may lead to hemorrhoids
o Hypertension, particularly portal hypertension, can also cause hemorrhoids
o Pregnancy may lead to hypertension and increase strain during bowel movements, so hemorrhoids are often associated with pregnancy
o Cigarette smoking during bowel movements, may worsen hemorrhoids and may lead to severe internal bleeding of veins inside re**um area

The pressure causes the normal a**l veins and tissue to swell. This tissue can bleed, often during bowel movements.

Symptoms of Haemorrhoids or Piles(Arsha)

• Blood in stools
• Pain while defecating
• Painful hard lump in the a**s
• Mucus discharge while defecating
• Itching sensation around the a**s
• Feeling of fullness in the bowels even after passing a stool
• Secondary anaemia

Main classification of Piles or Hemorrhoids(Arsha)

They can be classified into internal and external hemorrhoids. There is a dentate line in the a**l ca**l, which separates two types of a**l skin. While the skin above the dentate line is not sensitive to pain, the skin below this line is quite sensitive to pain.

1. Internal hemorrhoids occur above the dentate line and are, therefore, usually painless. They can cause bleeding or they may push out of (pr*****ed) the a**l opening when they become quite large.

2. External hemorrhoids occur below the dentate line and are generally painful. External hemorrhoids can become very distressing when the blood in the hemorrhoid clots and inflammation irritates the nearby pain-sensitive skin

Treatment for hemorrhoids (Arsha) should be carried out in four ways with each having its specific indications.

These are:
1. Medical management (Bheshaja chikitsa)
This mode of treatment can cure first degree hemorrhoids. In other degrees of piles it may be used along with other procedures. This treatment is done after assessing dosha predominance.

2. Operative management (Sastra chikitsa) – Application of Kshara-sutra Therapy.
In this process hemorrhoids are tied off at its base (pedicle) by Kshara-sutra or specially processed thread with medicines. By the principle ofpressure necrosis, it prevents blood supply to the respective pile mass immediately. Eventually hemorrhoids shrink, shrivel up, die and ultimately fall off within 10 days, leaving some scar tissue which supports the corresponding veins in that area, preventing them to distend or bulge further.

3. Application of Ayurvedic Kshara (Caustics made of Ayurvedic herbs)
It is a non-surgical procedure of Ayurveda indicated for the management of hemorrhoids. A medicine (alkaline in nature) derived from a combination of various herbs is applied to the pile mass with the help of a special slit proctoscope. It is a type of chemical cauterization. Application of kshara is considered superior over other procedures used for the treatment of hemorrhoids or piles.

4. Cauterisation or Agnikarma
Agnikarma in the form of thermal cautery is a suitable procedure in the excision of sentinel piles and a**l warts falling in Vatha Kaphaja type of Arsha.

Do’s and Don’ts for Hemorrhoids or Piles

Do’s

• Drink Plenty of fluids
• Do take adequate fibers in the diet
• Exercise regularly. Walking is the one of the best things
• Train yourself for regular food intake of lunch and dinner
• Lose weight, if you are overweight
• Chew your food properly which enable digestion
• Take buttermilk, onion, leafy vegetables and gree gram etc
• You should not eat any food which is hard to digest
• You should also cut back on spicy and oily foods
• Increase your intake of okra, spinach, carrots, radish and yam in your everyday menu

• You should take the medications as prescribed
• You should avoid lifting heavy weights for at least 5 to 7 days.
• Drink plenty of water and other fluids—at least 10 glasses a day.
• Eat foods, which are good sources of fibre, such as whole grain or bran, cereals and breads, fresh vegetables and fruits, and, if necessary, add bran to your foods (about three to four tablespoonfuls per day)
• Keep the a**l area clean
• Take warm baths
• The hot poultice of dry radish is very good in non-bleeding piles
• Have enough sleep

Don’ts

• Don’t sit too much on a hard surface because it can restrict blood flow around the a**l area
• Don’t sit in the toilet for long periods. This position causes extra pressure on the a**l area
• Don’t drink coffee or alcohol
• Hurry, worry and curry is absolutely not advisable for a piles patient
• Don’t lift heavy objects. Even if you lift anything heavy exhale and don’t hold your breath
• Don’t use laxatives regularly as it may become a habit and cause the bowels to lose its ability to function normally
• Don’t consume any medication directly; this can be harmful for your stomach as well as diseases

Indigestion or dyspepsia

uncomfortable sensation in your belly during or after a meal. You might have burning or pain in the upper part of your stomach, too. It’s indigestion, also called dyspepsia. Indigestion is a general term that describes a group of gastrointestinal symptoms that occur together, including pain, a burning feeling, or discomfort in your upper abdomen; feeling full too soon while eating; and feeling uncomfortably full after eating.

Causes

Men and women of all ages can get indigestion. It’s a common condition. But certain things make some people more prone to it. Causes include:
Diseases:

• Ulcers
• GERD
• Stomach cancer. This is rare.
• Gastroparesis, a condition where the stomach doesn't empty properly. It often happens to people with diabetes.
• Stomach infections
• Irritable bowel syndrome
• Pancreatitis, an inflamed pancreas
• Thyroid disease

Lifestyle:

• Eating too much, too fast, or when you’re stressed. High-fat foods can also add to the problem.
• Stress and fatigue

• Some of the causes of indigestion include
• drinking
o too many alcoholic beverages
o too much coffee or too many drinks containing caffeine
o too many carbonated, or fizzy, drinks
• eating
o too fast or too much during a meal
o spicy, fatty, or greasy foods
o foods that contain a lot of acid, such as tomatoes, tomato products, and oranges
• feeling stressed
• smoking
• Some medicines can cause indigestion, such as

Medications:

• Aspirin and many other pain relievers
• Estrogen and birth control pills
• Steroid medications
• Some antibiotics
• Thyroid medicines
• certain antibiotics—medicines that kill bacteria
• nonsteroidal anti-inflammatory drugs etc

 Sometimes people have long-lasting indigestion that isn’t related to any of these things. This type is called functional or non-ulcer dyspepsia.
 Many women have indigestion during the middle and later parts of pregnancy. The problem might come from hormones, which relax the muscles of the digestive tract, and from the pressure the growing baby puts on the stomach.


How Can I Prevent Indigestion?

The best way to avoid getting it is to steer clear of the foods and situations that seem to cause it. You can keep a food diary to figure out what you eat that gives you trouble. Other ways to prevent the problem:
• Eat small meals so your stomach doesn’t have to work as hard or as long.
• Try not to chew with your mouth open, talk while you chew, or eat too fast. This makes you swallow too much air, which can add to indigestion.
• Avoid late-night eating.
• Drink beverages after rather than during meals.
• Eat slowly.
• Avoid foods with a lot of acid, such as citrus fruits and tomatoes.
• Limit spicy foods
• Limit fried and greasy foods
• Cut back on or avoid foods and drinks that have caffeine.
• If you smoke, quit. Or at least, don’t light up right before or after you eat, since smoking can irritate your stomach.
• Cut back on alcohol.
• Avoid wearing tight-fitting clothes. They can put pressure on your stomach, which can make the food you’ve eaten move up into your esophagus.
• Don't exercise with a full stomach. Do it before a meal or at least 1 hour after you eat.
• Don't lie down right after you’ve eaten.
• Wait at least 3 hours after your last meal of the day before you go to bed.

Raise the top of your bed so that your head and chest are higher than your feet. You can do this by placing 6-inch blocks under the top bedposts. Don't use piles of pillows to achieve the same goal. You’ll only put your head at an angle that can increase pressure on your stomach and make heartburn worse.
You might not need any treatment at all. Indigestion often goes away on its own after a few hours. But let your doctor know if your symptoms get worse.
If you don’t feel better after these changes, your doctor may prescribe medications for you.

Vitiligo or Leucoderma

Vitiligo is a progressive , idiopathic , pigmentation disorder of the skin characterized by white lesions.
In ayurveda SHVITRA or KILASA is the term to describe hypopigmentation disorder of skin
It is an acquired de-pigmentation of the skin that is very common with a worldwide preponderance. Leucoderma or vitiligo is a distressful skin disease and the word Leucoderma means ‘to have white skin’. There would be a gradual loss of pigment called melanin from the dermal layers that results into white patches. These patches look bad, especially in persons having dark complexions. Leucoderma can occur at any age, person and in any kind of skins.

Leucoderma is a fairly common disorder affecting nearly 2% of the world’s population.

The incidence is a little more eminent in India. Leucoderma is, nevertheless, more common in women as compared to men. The most affected areas include the hands, the neck, the back and the wrists in the order.
Leucoderma is neither infectious nor contagious, which starts with a small white or pale colored patch and enlarges into larger patch in course of time. It has a considerable psycho-social impact.

Symptoms

The various symptoms observed during Leucoderma are:
• A small, white patch on the skin which may expand with time
• Premature greying of hair
• Loss of hair
• Hair on the patches become white
• Sensitivity towards cold
• The patient may often suffer from mood swings or depression
• Loss of colour inside the mouth

Causes

There are many theories as to what may be responsible for causing leucoderma. Researchers have suggested an auto-immunological, neurological or auto-cytotoxic origin. None of these have been proven definitive.
However, According to the medical experts, the common causes of leucoderma are:
• Chronic or acute gastric diseases
• Deficiency of calcium
• Inflammatory skin conditions
• Burns
• Intralesional steroid injection
• Postdermabrasion
• Excessive stress, tension and worry
• Impairment of hepatic or liver function (due to jaundice), worm infestation
• Wearing tight clothes
• Wearing rubber gloves
• Using stickers or tattoos on the skin

DIET AND LIFESTYLE

When we try to control vitiligo , the first line of management is to detox our system. We need to prevent further accumulation of toxic in our system for which diet plays a very important role , so avoid all process of frozen foodstuffs from your daily intakes and make sure to take a lot of fruits nuts and try to take lot of water
 Try to take some bitter items in taste like a bitter guard, drumsticks and other to prevent the imbalance.
 Make some distance from the foods that are difficult to digest along sour taste ,excessive salt, curd, fish, jaggery and sesame seeds.
 Avoid consumption of opposite foods like fish and milk together
 Have sound and adequate sleep daily
 Do not come in direct dun light and cold and if necessary then apply sun screen cream to the area mainly exposing to the sun.
 Try following diet and routine according toyour vaidya or doctor

Ayurveda Management

In Ayurveda, it is referred as Shwetakushta or Switra or Kilasa and classified into Kushta rogas (skin diseases). It is caused by the imbalance of all the three doshas (Vata, Pitta & Kapha) vitiating Rakta, Mamsa and MedhasDhatus.
Switra is classified into following types:
• Daruna – when doshas vitiates the rakta dhatu (blood), the patches will be red in color.
• Aruna – when doshas vitiates the mamsa dhatu (muscle tissue), the patches will be coppery in color.
• Kilasa – when doshas vitiates the medho dhatu (fat tissue), the patches will be white in color. This is the most common clinical presentation of svitra hence leucoderma is often called Kilasa.
Ayurveda advocates following line of treatment:
• Shodana chikitsa – Strong detoxification Panchakarma procedures should be done. Vamana, Virechana, Rakta mokshana using leeches are recommended.
• Shamana Chikitsa – done through oral medications and external applications.
• Patya (Diet) – strict diet restrictions, Lifestyle modifications, Yoga and meditation are advocated.
Apart from these Ayurveda has also given importance to Prayachitham (actions which remove sins) for the Paapa Karmas (sinful acts).
we are providing a very effective therapy for Leucoderma with above said guidelines and researched medicines with high treatment success rate. Early intervention of treatment (at least within 1 year from onset) gives maximum results.
Vitiligo or Switra has an unpredictable course and that its prognosis may not always be encouraging. One must avoid physical, chemical, and emotional trauma as far as possible. 1 year crossed and the lesions appearing in palm, sole, genitalia and lips are difficult to cure.
